目的 收集、总结解放军第159医院烧伤病区1年内的细菌流行状况及药敏实验结果,为临床选择合理抗菌药物提供参考。方法 对2010年3月~2011年3月间,1050株烧伤患者创面细菌培养及药敏试验结果进行回顾性收集、整理和分析、总结。结果 在分离的57种1050株细菌中,革兰阴性(G-)杆菌35中654株占62.29%,革兰阳性(G+)球菌17中342株,占32.57%,真菌5种54株,占5.14%。抗菌药物敏感率:泛耐药铜绿假单胞菌对舒普深的敏感率为46.67%(中介);鲍曼不动杆菌对舒普深的敏感率为96.5%,但泛耐药鲍曼不动杆菌仅对舒普深敏感,且敏感率只有34.5%;奇异变形杆菌对亚胺培南、舒普深的敏感率均为100%;洛菲不动杆菌对舒普深的敏感率为98.59%;革兰阳性菌对万古霉素的敏感率为100%;白假丝酵母菌和毛霉对氟康唑、酮康唑的敏感率100%;黄曲霉菌对伊曲霉菌和克霉唑的敏感率为94.12%;结论 烧伤病区细菌以革兰阴性菌为主,条件致病菌和真菌较之前增加,细菌耐药问题严重,合理使用抗菌药物,正确处理烧伤创面,早日封闭创面是应对感染和细菌耐药的有效措施。 |

Objective: To collect and summarize the bacterial epidemic status and results of drug sensitivity within the burn ward area of No. 159 Hospital of PLA in order to provide a reference for selecting an appropriate anti-bacterial agent. Methods: The wound culture and drug sensitivity results of 1050 bacterium on the burn wound from March 2010 to March 2011 were retrospectively collected, analyzed and summarized. Results: Among the 57 types bacteria of 1050 bacterium, there were 35 types gram negative bacterolis (654, 62.29%) ,17 types of gram positive coccus (342, 32.57%) and 5 types of fugus (54, 5.14%). Sensitivity rate of anti-bacterial agents: the sensitivity of PDRPA to iperazon was 46.67%; the sensitivity of baumnnii to iperazon was 96.5%. But PDRAB was only sensitive to iperazon with a sensitivity of 34.5%. Proteus mirabilis had 100% sensitivity to impinenem and iperazon. Acinetobacter lwoffii had 98.59% sensitivity to iperazon. The sensitivity of gram positive bacterium to vancomycin was 100%. The sensitivity of candida albicans and mucor to fluconazole and ketoconazole was 100%. The sensitivity of aspergillus flavus to itramycin and clotriamazole was 94.12%. Conclusion: The vast majority of bacteria in the burn wound are gram negative bacterium. Conditional pathogens and fungi were more than before. Drug resistance was serious. It is effective to manage infection and drug resistance with appropriate anti-bacterial agents and correct wound management as well as close the wound as early as possible. |
