| 规范的MEBT/ MEBO 创面换药技术可以减轻患者痛苦、缩短创面愈合时间、提高创面愈合质量。创面处理原则为早期、全程、规范换药, 依创面深度不同而采用不同的处理方法。具体为: 1． 深Ⅱ度烧伤创面: 创面愈合后使用疤痕软膏联合弹力绷带加压包扎1 年左右预防瘢痕; 2． Ⅲ度浅型烧伤创面: 伤后即刻进行耕耘减张, 大面积烧伤患者需在生命体征平稳的状态下才可实施耕耘减张处理, 但一般应在48 h内; 创面愈合后使用疤痕软膏联合弹力绷带加压包扎1 年半左右预防瘢痕; 3． Ⅲ度深型烧伤创面: 在伤后24 h内进行耕耘减张、薄化处理, 耕耘减张后第2 天对创面进行开窗, 逐步清除坏死组织。创面愈合后使用疤痕软膏和弹力绷带加压包扎2 年左右预防瘢痕。在规范的MEBT/ MEBO 治疗下, 创面的愈合标准为深Ⅱ度深型以下创面无瘢痕愈合; 浅Ⅲ度创面无瘢痕或表浅瘢痕愈合, 无功能障碍。下图是Ⅲ度深型创面规范换药的典型图例, 详细病例资料请观看本期杂志所附光盘。 |

| Standardized dressing change techniques of MEBT/ MEBO can relieve patients'pain, shorten the healing time and improve the wound healing quality. The principles of wound management are early application of MEBT/ MEBO through the whole treatment course, following standard procedures to change dressing and adopting different management ap- proaches based on wound depths. And the details are as follows: 1. Deep Ⅱ degree burns: after the wound healing, pressu- ring bandage the wound by using elastic bandage after applying MEBO Scar Ointment for about one year to prevent scar forma- tion; 2. Ⅲ degree superficial burns: skin incision and tension relieving technique should be performed immediately after the injury, but for the extensive burns, it should be adopted under the conditions of stable vital signs, and generally within 48 hrs. After the wound healing, pressuring bandage the wound by using elastic bandage after applying MEBO Scar Ointment for about one and a half years to prevent scar formation; 3. Ⅲ degree deep burns: perform the skin incision and tension relieving tech- nique and the eschar thinning technique within 24 hrs after the injury. And at the second day after that, perform the windowing technique on the wound to remove the necrotic tissues gradually. After the wound healing, pressuring bandage the wound by u- sing elastic bandage after applying MEBO Scar Ointment for about two years to prevent scar formation. Under the premise of standardizing application of MEBT/ MEBO, the evaluation criteria of wound healing are that the wounds below deep II degree deep type heal without scar formation and the wounds of superficial Ⅲ degree heal with no scar or superficial scar but with no dysfunction. The pictures below are the typical cases of deep Ⅲ degree burns treatment following the standard dressing change procedures.
