目的观察湿润烧伤膏（MEBO）联合负压辅助植皮修复臀部压疮的效果。方法 19例老年臀部压疮患者在应用MEBO治疗至基底肉芽组织生长良好后，局麻下进行皮片移植修复创面，根据皮片固定方式将19例患者随机分为实验组（10例）和对照组（9例)，实验组用改良型密闭式负压［1］辅助固定移植皮片；对照组常规加压包扎固定移植皮片。对比观察两组患者压疮创面皮片存活率及创面愈合时间。结果术后第1次启视创面实验组皮片存活率为95.30%±3.38%，对照组皮片存活率为56.5%±8.45%，经t检验，P<0.01，差异具有统计学意义；术后第2次启视创面实验组皮片存活率为90.20%±7.05%，对照组皮片存活率为26.45%±5.62%，经t检验，P<0.01，差异具有统计学意义；；实验组创面愈合时间为41.36 d±8.07 d，对照组创面愈合时间为75.72 d±12.56 d，经t检验，P<0.01，差异具有统计学差异。结论压疮创面经MEBO换药治疗至肉芽组织生长良好后，行皮片移植修复创面时，采用负压辅助固定移植皮片可提高皮片存活率，促进创面愈合，值得临床推广。

英文摘要:

Objective Toobserve the efficacy of MEBO combined with vacuumassistedskin grafting intreatingbuttock bedsore.Methods Nineteen frail elderly patients with buttock bedsores were treated with MEBO until the granulation tissues grew well, then skin grafting was done under local anesthesia to repair the wound.Nineteen patientswere randomly divided into experimental group (10 cases) and control group (9 cases): In experimental group, improved closed vacuumassistedfixing the grafted skin flaps; In control group, routine bandaging was used tofix the grafted skin flaps. The survival rate of skin flaps on the bedsore woundsand the wound healing time were comparatively observed. Results After operation, the first wound inspection showed skin flap survival rate of 95.30%±3.38% in experimental group and 56.5%±8.45% in control group，ttest gave the result of P<0.01，indicating a difference of statistical significance；the second wound inspection showed skin flap survival rate of 90.20%±7.05% in experimental group and 26.45%±5.62% in control group，ttest gave the result of P<0.01，indicating a difference of statistical significance； The wound healing time was 41.36 d±8.07d in experimental group and 75.72 d±12.56d in control group, ttest gave the result of P<0.01，indicating a difference of statistical significance Conclusions When bedsore woundsare treated with MEBO until the granulation tissues grow well and then skin grafting is done to repair the wounds, vacuumassisted fixing the grafted skin flapscan improve the survival rate of skin flaps, promote the wound healing, so this methods is worthy to be promoted in clinic.
