          目的 总结2011-2013年中南大学湘雅三医院烧伤科病房致病菌的分布及耐药情况,为临床提供最新的流行病学资料,以优化临床用药方案。方法 收集2011年1月-2013年12月烧伤科送检的1240例患者的2025份标本的细菌培养及药敏检测资料,并与1995-2000年烧伤科送检标本的细菌培养及药敏检测结果进行对比分析。结果 近3年烧伤科检出l病原菌以革兰阴性菌为主,检出率最高者为铜绿假单胞菌,其次为鲍曼不动杆菌、金黄色葡萄球菌。铜绿假单胞菌对多黏菌素B耐药率最低,为48.4%;鲍曼不动杆菌对替加环素耐药率最低,为48.8%;金黄色葡萄球菌对万古霉素耐药率最低,为5.3%。与1995-2000年的检测结果相比,革兰阴性菌检出率呈上升趋势,革兰阳性菌及真菌检出率呈下降趋势,且优势菌及其抗菌谱变化较大。结论 中南大学湘雅三医院烧伤科病房近年来的细菌学分布有所变化,整体上呈现出多重耐药、高耐药性的特征,对临床控制和治疗耐药菌株感染提出了严峻的考验。

          Objective Sum up the distribution of the pathogenic bacteria and the situation of antibiotic resistance during 2011-2013 in wards of the Burns Department of the Third Xiangya Hospital of Central South University to provide the latest epidemiological data, so as to optimize the regimen of clinical treatment. Method The data of bacteria culture and drug resistance profile of 2025 specimens from 1240 patients of burns department during 2011 to 2013, was collected and analyzed in comparison with the counterpart data of 1995-2000. Results The dominant detected bacteria in burns department was Gram negative bacteria in recent 3 years. The top one is pseudomonas aeruginosa, followed by acinetobacter baumannii and staphylococcus aureus. The lowest drug resistant ratio is: Pseudomonas aeruginosa to Polymyxin B, 48.4 %; Acinetobacter baumannii to Tigecycline, 48.8%; Staphylococcus aureus to vancomycin, 5.3%. Compared with 1995-2000 data, the detection rate is rising for Gram negative bacteria and lowering for Gram positive bacteria and fungus, and the dominant bacteria and its antibacterial spectrum changed a lot. Conclusion The distribution of bacteria in the wards of the Burns Department of the Third Xiangya Hospital of Central South University has changed in recent years, featuring multi-drug resistance and high resistance which is a big challenge for the control and cure of infection of drug resistant strains.
