目的 研究湿润烧伤膏对SD大鼠深Ⅱ度烫伤创面的影响及作用机制。方法 将60只SD大鼠制成深Ⅱ度烫伤模型,并按照随机数表法分为湿润烧伤膏组(30只),创面外用湿润烧伤膏换药治疗;磺胺嘧啶银组(30只),创面外用磺胺嘧啶银换药治疗。于烫伤后第3 天和第7天通过免疫组化法观察CD31、Ki67在创面上的表达,作为创面肉芽组织增生情况的评判指标;测定大鼠烫伤创面愈合率,作为判断创面愈合情况的指标;采用荧光定量PCR法测定两组大鼠血小板源性生长因子及表皮生长因子的表达量。结果 以愈合率达到95%为创面愈合,实验组创面平均愈合时间为17d,对照组创面平均愈合时间为20d,两组创面愈合率进行配对t检验,P<0.05,差异具有统计学意义。免疫组化法观察结果显示,实验组CD31及Ki68的表达均强于对照组。Real-Time PCR 结果显示,实验组PDGF(血小板源性生长因子)在伤后1-7d的表达量均强于对照组;EGF(表皮生长因子)在伤后3-10d的表达量也均强于对照组。结论 湿润烧伤膏能通过增加PDGF、EGF的表达促进创面血管新生及细胞增殖,加速上皮化进程,达到防止创面加深、促进创面愈合的作用。 |

Objective To study the mechanism and effect of MEBO in treatment of SD rats with deep second degree scald wounds. Methods Used 60 SD rats to establishdeep second degree scald models and randomly divided them into two groups to observe the scald healing rate: 30 rats in the experimental group were treated with MEBO while sulfadiazine silver was used in the control group of 30 rats. Used immunohistochemical method to observe the expression of CD31 and Ki67 o\in the scald wounds on the 3rd day and 7th day of the treatment, the index of which is the evaluation of the wound granulation tissue growth condition; wound healing rate of the SD rats scald was measured as the index of evaluating the wound healing condition; FQ-PCR method was used to measure the expression quantity of platelet-derived growth factor (PDGF) and epidermal growth factor (EGF). Results Taking the healing rate of 95% or more as wound healed, and the average healing time was 17 days in experimental group, and 20 days in control group. According to the t pair test of the wound healing rate in both groups, P<0.05, the difference had statistical significance. Immunohistochemical examination result showed that the expression of CD31 and Ki 68 of the experimental group was stronger than the control group. Real-Time PCR result showed that PDGF expression quantity of the experimental group was stronger than the control group within 1~7 days after injuries; EGF expression quantity of the experimental group was stronger than the control group within 3~10 days after injuries. Conclusion By increasing the expression of PDGF and EGF, MEBO can promote angiogenesis and cell proliferation, and accelerate epithelization process, so as to prevent wound deepening and achieve the rapid wound healing. |
