【摘要】 目的 观察甘草酸二铵防治重度烧伤患者肝功能异常的临床疗效?方法 将2012 年3 月- 2014年12 月邢台市人民医院收治的30 例重度烧伤患者按随机数表法随机分为治疗组(15 例) 和对照组(15 例),对照组患者给予常规补液治疗, 治疗组患者在常规补液治疗的基础上加用甘草酸二铵治疗?分别于伤后即刻及伤后第3?7?14?21?28 天上午7 时抽取患者静脉血, 检测丙氨酸氨基转移酶(ALT)?天冬氨酸氨基转移酶(AST) 及L?γ?谷氨酰转肽酶(γ?GT), 观察两组患者肝功能变化情况?结果 两组患者烧伤后不同时间段肝功能变化情况对比, 同一时间段治疗组血清ALT?AST?γ?GT 值始终低于对照组, 即便是在治疗后7 d 内, 血清ALT?AST?γ?GT 值均升高的情况下, 治疗组的升高程度也明显低于对照组, P < 0?? 01, 差异具有统计学意义?结论 重度烧伤患者早期预防性应用甘草酸二铵可有效防治肝功能异常, 值得临床推广应用? |

【Abstract】 Objective To observe the clinical efficacy of Diammonium Glycyrrhizinate in the prevention and treatment of liver dysfunction in patients with severe burns. Methods Thirty severely?burned patients admitted into Xingtai People’s Hospital between March 2012 and December 2014 were evenly divided into a treatment group and a control group according to the random number table. The control group was treated with routine fluid infusion while the treatment group was treated with Diammonium Glycyrrhizinate on the basis of routine fluid infusion. Venous blood of patients was collected immediately after the injury and at 7∶ 00am on day 3, 7, 14, 21 and 28 after the injury to test alanine aminotransferase (ALT), aspartate aminotransferase (AST) and glutamyltranspeptidase (γ?GT), and the functional changes of liver in both groups were observed. Results The comparison of post?burn liver functional changes at different periods in the two groups showed the values of serum ALT, AST and γ?GT in the treatment group were always lower than that in the control group at the same period. Even within the first 7 days after the treatment when the values of serum ALT, AST and γ?GT were all increasing, their values in the treatment group were obviously lower than that in the control group, showing statistically significant difference, P <0?? 01, which represented that the combined application of Diammonium Glycyrrhizinate on the basis of fluid infusion can significantly prevent and treat liver dysfunctions. Conclusion The early preventive application of Diammonium Glycyrrhizinate in severely?burned patients can effectively prevent and treat the functional abnormalities of liver,being worthy of clinical application and popularization. |
