          【摘要】 目的 观察717解毒合剂联合西药治疗对蝮蛇咬伤患者血清乳酸脱氢酶(LDH)、肌酸激酶(CK)及肌酸激酶同工酶(CK-MB)的影响。方法 将2016年5月—2016年10月江西中医药大学附属医院中医外科收治的60例蝮蛇咬伤患者随机分为治疗组(30例)与对照组(30例),其中对照组患者予以常规西药治疗,治疗组患者在对照组治疗的基础上加服717解毒合剂,对比观察两组患者的临床疗效及治疗前后患者血清LDH、CK及CK-MB的含量变化。结果 治疗组患者总有效率为93.33%,明显优于对照组的70%,P<0.05,差异具有统计学意义;治疗7 d后,两组患者血清LDH、CK及CK-MB含量较治疗前均显著下降,且治疗组下降更为明显,P<0.05,差异具有统计学意义。结论 717解毒合剂与抗蝮蛇毒血清联合治疗蝮蛇咬伤,既能快速中和患者血清内的游离毒素,又能有效降低结合毒素对患者心肌细胞及骨骼肌细胞等的损害,从而显著降低患者血清LDH、CK及CK-MB含量,提高蝮蛇咬伤患者的治愈率,值得临床推广应用。

          【Abstract】Objective To observe the effects of 717 antidote mixture in combination with western medicine on the levels of lactate dehydrogenase (LDH), Creatine Kinase (CK) and CK-MB in Agkistrodon snakebite patients. Methods Sixty Agkistrodon snakebite patients admitted to TCM Surgery Department of The Affiliated Hospital of Jiangxi University of TCM between May 2016 and October 2016 were randomly distributed into a treatment group (30 patients) and a control group (30 patients). Patients in the control group were treated by the conventional western medicine therapy while patients in the treatment group were treated by 717 antidote mixture in addition to the conventional western medicine therapy. The clinical efficacy and the serum levels of LDH, CK and CK-MB before and after the treatment were then observed and compared between the two groups. Results The total effective rate in the treatment group was 93.33% which was statistically significantly higher than that in the control group (70.00%) (P<0.05); after 7 days of treatment, the serum levels of LDH, CK and CK-MB in the two groups were both significantly lower than that before the treatment. However, the decreases of the serum biomarkers in the treatment group were more significant compared to the control group and the differences were statistically significant with all P<0.05. Conclusion Treating Agkistrodon snakebites with 717 antidote mixture combined with Agkistrodon antivenin can rapidly neutralize the free venins in the serum and, at the same time, reduce the damage to cardiomyocytes and skeletal muscle cells caused by bound venins, thereby significantly lowering the serum levels of LDH, CK and CK-MB and increasing the cure rate of Agkistrodon snakebites, and this treatment can be promoted in clinical practice.
