          【摘要】 目的 研究探讨皮肤再生医疗技术(moist exposed burn therapy/moist exposed burn ointment, MEBT/MEBO) 对大鼠慢性难愈合创面组织中表皮生长因子受体(epidermal growth factor receptor, EGFR) 表达的影响?方法 将60 只SPF 级SD 雄性大鼠随机分为空白组(12只)?对照组(12只)?模型组(12 只)?贝复新组(12只) 及MEBO 组(12 只), 其中, 空白组大鼠做背部备皮处理后采用生理盐水纱布湿敷; 对照组大鼠做深达筋膜层的全层皮肤缺损创面模型后采用生理盐水纱布覆盖治疗; 模型组大鼠做慢性难愈合创面模型后采用生理盐水纱布覆盖治疗; 贝复新组大鼠做慢性难愈合创面模型后采用贝复新治疗; MEBO 组大鼠做慢性难愈合创面模型后采用MEBO 治疗?分别于治疗第3、7、14天取各组大鼠相同部位皮肤或创面组织进行HE 染色, 并用Western blot法检测组织中EGFR 的含量, 对比观察各组大鼠皮肤或创面组织中EGFR 的表达水平及组织病理变化?结果 局部处理后第3、7天, 除空白组外, 其余各组大鼠创面组织HE 染色病理切片均显示炎症细胞增多, 有新生血管生成; 局部处理后第14 天, 对照组?贝复新组?MEBO 组大鼠创面组织中炎症细胞消失, 创面再上皮化?局部处理后第3?7?14 天, 空白组大鼠背部皮肤组织中EGFR 表达水平无明显变化, F =0.667, P>0.05, 差异无统计学意义; 对照组?模型组?贝复新组及MEBO 组大鼠创面组织中EGFR 表达水平变化均较明显, F 值分别为79.546、89.600、63.036、56.628, P均<0.01, 差异具有统计学意义, 其中对照组?贝复新组及MEBO 组大鼠创面组织中EGFR 表达水平均呈先升高后降低的趋势, 而模型组则呈递增趋势; 局部处理后第3、7、14 天,各组大鼠皮肤或创面组织中EGFR 表达水平组间两两对比, 除贝复新组与MEBO 组对比, P> 0.05, 差异无统计学意义外, 其余各组组间两两对比, P均< 0.05, 差异具有统计学意义; 局部处理后第3?7?14 天, 各组大鼠皮肤或创面组织中EGFR 表达水平多个样本均数对比, F 值分别为57.036、134.368、92.829, P 均<0.01, 差异具有统计学意义?结论 MEBT/ MEBO 可通过上调大鼠慢性难愈合创面组织中EGFR 的表达水平, 促进慢性难愈合创面的愈合, 能够为MEBT/ MEBO 治疗慢性难愈合创面的临床应用提供理论基础?

          【Abstract】Objective To investigate the effect of Moist exposed burn therapy/Moist exposed burn ointment (MEBT/MEBO) on the expressions of EGFR in chronic refractory surface wounds of rats. Methods 60 Sprague-Dawley (SD) male rats were randomly divided into 5 groups (Blank group, Control group, Model group, rb-bFGF group, and MEBO group, and each group has 12 rats). Back skin preparation was done on rats in blank group and then covered with wet saline gauze; full-thickness skin defect wound model that reached the depth of fascia was made on rats in control group and then covered with saline gauze; chronic refractory wound model was made on rats in model group and then covered with saline gauze; chronic refractory wound model was made on rats in rb-bFGF group and then covered with rb-bFGF gauze; Chronic refractory wound model was made on rats in MEBO Group and then covered with MEBO gauze. On day 3, 7 and 14 after treatment, skin or wound tissues in the same part were obtained and HE staining was performed on them and protein expression of EGFR was detected by the Western blotting analysis. The expression levels of EGFR and histopathological changes in skin or wound tissues of the rats in all groups were compared and observed. Result On day 3 and 7 after local treatment, except blank group, the histopathological slices of the rats in all other groups showed an increase in inflammatory cells and angiogenesis was observed. On day 14 after local treatment, inflammatory cells in control group, rb-bFGF group and MEBO group disappeared and epithelization?was found on wounds. On day 3, 7, 14 after local treatment, no significant change in expression level of EGFR in back skin tissues of rats in blank group was found and the difference was statistically not significant(F=0.667, P>0.05). Significant changes in expression levels of EGFR in back skin tissues of rats in blank group, model group, rb-bFGF group, and MEBO group were found and the differences were statistically significant(F=79.546, 89.600, 63.036 and 56.628, P<0.01). The expression level of EGFR in skin or wound tissues of rats in control group, rb-bFGF group and MEBO group showed a trend of increase first and then decreased and it only increased in model group. On day 3, 7, 14 after local treatment, comparison of expression level of EGFR in skin or wound tissues of rats between two group was conducted and the result showed statistically significant difference(P<0.05), except the comparison between rb-bFGF group and MEBO group, which showed no statistically significant difference(P>0.05). On day 3, 7, 14 after local treatment, means of multiple samples of expression level of EGFR in skin or wound tissues of rats between two groups were compared and the result showed statistically significant difference(F=57.036,134.368 and 92.829,P<0.01). Conclusion MEBO can promote wound healing by up-regulating EGFR expression in granulation tissues of chronic refractory surface wound, so it provides a theoretical basis for clinical application of MEBO/MEBT in treating chronic refractory wounds.
