| 【摘要】 目的 对比观察强脉冲激光联合压力疗法治疗深度烧伤增生性瘢痕的临床疗效?方法 将2015年4月—2017年4月中国人民解放军海军军医大学附属长海医院烧伤科收治的112 例深度烧伤增生性瘢痕患者按照随机数表法随机分为观察组(56例) 与对照组(56例), 其中观察组患者采用强脉冲激光联合压力疗法治疗,对照组患者单纯采用压力疗法治疗, 对比分析两组患者治疗6个月后的总有效率, 并分别采用温哥华瘢痕量表(vancouver scar scale, VSS) 和中文版精简烧伤健康量表(burn specific health scale-brief, BSHS-BB) 评估两组患者治疗前后的瘢痕情况及生活质量?结果 治疗6个月后, 观察组患者的总有效率为92.86%, 对照组患者的总有效率为67.86%, 两组对比采用等级资料的秩和检验, 校正u=4.029, P< 0.01, 差异具有统计学意义; 治疗6个月后, 观察组患者的VSS 评分为(1.29±0.51) 分?BSHS-B总评分为(313.19±43.86) 分, 对照组患者的VSS评分为(5.69±0.72)分?BSHS-B总评分为(263.01±47.32)分, 两组对比采用u检验, u值分别为37.318及5.820, P均<0.01, 差异具有统计学意义?结论 强脉冲激光联合压力疗法治疗深度烧伤增生性瘢痕,可有效淡化色素沉着, 明显改善或消除增生性瘢痕的临床症状, 提高治疗总有效率和患者生活质量, 值得临床推广应用? |

| 【Abstract】 Objective To observe comparatively the clinical effect of intense pulsed laser combined with compression therapy on hyperplastic scar of deep burn. Methods 112 deep burn patients with hyperplastic scars, admitted to Burns Department, Changhai Hospital from April 2015 to April 2017, were randomly divided into observation group( 56 cases) and control group(56 cases).Patients in observation group were given the treatment of intense pulsed laser combined with compression therapy and patients in control group were given compression therapy only. The total effective rate for patients in the two groups after 6 months of treatment were compared and analyzed. VSS (vancouver scar scale) and BSHS-B(burn specific health scale-brief) were used to evaluate the patients’ life quality and scar condition. Results After 6 months of treatment, the total effective rate was 92.86% for observation group and 67.86% for control group. The rank-sum test for ranked data was used for the comparison of the two groups and the results showed statistically significant difference(corrected u = 4.029 and P＜0.01 ); after 6 months of treatment, VSS score and BSHS-B score for observation group were （1.29±0.51）points and （313.19±43.86）points respectively and for control group, they were （5.69±0.72）points and （263.01±47.32）points respectively. u test was used for the comparison of the two groups and results showed statistically significant difference(u= 37.318 and 5.820,and P＜0.01). Conclusion In the treatment of hyperplastic scar of deep burn, intense pulsed laser combined with compression therapy can effectively lighten pigmentation, significantly reduce or eliminate the clinical symptoms of hyperplastic scar and improve total effective rate and patients’ life quality. It deserves to be promoted in clinical practice. |
