| 【摘要】 目的 分析探讨自体富血小板血浆治疗供皮区创面的临床疗效。方法 将2016年6月—2017年6月解放军四零一医院骨科收治的90例需自体表皮移植治疗的患者按照随机数表法随机分为试验组（45例）和对照组（45例），其中试验组患者于皮片切取后采用自体富血小板血浆联合凡士林油纱治疗供皮区创面，对照组患者于皮片切取后单纯采用凡士林油纱治疗供皮区创面。对比观察两组患者治疗14 d时的临床疗效以及供皮区创面的疼痛消失时间及愈合时间。结果 治疗过程中，两组患者的供皮区创面疼痛程度均逐渐减轻至完全消失，其中试验组患者的疼痛消失时间为（4.16±1.24）d，对照组患者的疼痛消失时间为（5.40±2.51）d，两组对比方差不齐，采用t’检验，t’=2.971，P<0.01，差异具有统计学意义；治疗14 d后，试验组患者的治疗总有效率为95.56%，明显优于对照组患者的治疗总有效率68.89%，两组对比采用等级资料的秩和检验，mann-whitney U=643.000，Z=-3.283，P=0.001，P<0.01，差异具有统计学意义；最终两组患者供皮区创面均完全愈合，且无不良反应发生，其中试验组患者的供皮区创面愈合时间为（11.11±1.41）d，对照组患者的供皮区创面愈合时间为（15.42±2.25）d，两组对比方差不齐，采用t’检验，t’=10.889，P<0.01，差异具有统计学意义。结论 自体富血小板血浆可有效缓解供皮区创面疼痛，促进创面愈合，疗效显著，且无严重不良反应发生，是一种较为理想的供皮区创面治疗方法，值得临床推广应用。 |

| 【Abstract】 Objective To analyze the clinical efficacy of autologous platelet-rich plasma in the treatment of donor site wounds.Methods Ninety patients with the necessity of autologous epidermal graft，admitted into the Orthopedics Department of The 401 Hospital of People’s Liberation Army from June 2016 to June 2017，were randomly divided into an experiment grouP（45 cases）and a control grouP（45 cases）according to the random number table.The donor site wounds were treated with autologous platelet-rich plasma combined with Vaseline gauze in the experiment grouPwhereas the donor site wounds in the control grouPwere treated simply with Vaseline gauze.The clinical efficacy，pain duration time and healing time of the donor site wounds were compared between the two groups on day 14 of treatment.Results During the treatment course，the wound pain on the donor sites gradually reduced and then completely disappeared in the two groups.The pain duration time was（4.16±1.24）d in the experiment group，which showed statistically significant difference from（5.40±2.51）d in the control grouPas compared by the t’test due to unequal variance，t’=2.971，P<0.01；After 14 days of treatment，the total effective rate in the treatment grouPwas 95.56%，which was significantly superior to 68.89% in the control group，and the rank sum test for ranked data was used for the comparison of the total effective rate between the two groups and the results showed statistically significant difference（mann-whitney U=643.000，Z=-3.283，P=0.001，P<0.01）；In the both groups，the donor site wounds were all completely healed and no adverse reactions ever occurred.The wound healing time in the experiment grouPwas（11.11±1.41）d while that in the control grouPwas（15.42±2.25）d，between which，due to unequal variance，t’test was used for the comparison and the difference was statistically significant（t’=10.889，P<0.01）.Conclusion Autologous platelet-rich plasma can effectively alleviate wound pain on the donor site and promote the wound healing，and will not lead to serious adverse reactions during the treatment course，and thus，it can be regarded as a much better treatment method for donor site wounds and should be promoted in clinical practice. |
