| 【摘要】 目的 对比观察吻合器痔上黏膜环切术（procedure for prolapse and hemorrhoids，PPH）联合温针灸及益肠通秘汤治疗老年出口梗阻型便秘的临床疗效。方法 将2016年1月—2016年12月湖北省中医院肛肠科收治的80例老年出口梗阻型便秘患者按照随机数表法随机分为试验组（43例）与对照组（37例），其中试验组患者采用PPH联合温针灸及益肠通秘汤治疗，对照组患者采用PPH联合乳果糖口服溶液治疗，对比观察两组患者治疗1个月后的排便情况、临床疗效及治疗后1年的复发情况。结果 治疗1个月后，试验组患者排便时间、排便频率、便后不尽感及排便顺畅度评分分别为（1.30±0.96）分、（1.35±0.95）分、（1.53±0.85）分、（1.40±0.93）分，对照组患者排便时间、排便频率、便后不尽感及排便顺畅度评分分别为（1.89±0.87）分、（2.05±1.10）分、（1.95±0.88）分、（1.84±0.99）分，两组对比，P均<0.05，差异具有统计学意义；治疗1个月后，试验组患者的治疗总有效率为95.75%，对照组患者的治疗总有效率为78.38%，两组对比，P<0.05，差异具有统计学意义；治疗后1年，试验组33例痊愈患者中出现复发者1例，对照组20例痊愈患者中出现复发者5例，两组对比，P<0.05，差异具有统计学意义。结论 PPH联合温针灸及益肠通秘汤治疗老年出口梗阻型便秘可明显缓解患者的便秘症状，降低复发率，疗效显著，值得临床推广应用。 |

| 【Abstract】 Objective To compare the therapeutic effect of procedure for prolapse and hemorrhoids（PPH）combined with needle warming moxibustion and Yichangtongmi decoction in the treatment of outlet obstructive constipation in the elderly.Methods Eighty elderly patients with outlet obstructive constipation，admitted into the Anorectal Department of Hubei Provincial Hospital of Traditional Chinese Medicine from January 2016 to December 2016，were randomly divided into an experiment group（43 cases）and a control group（37 cases）according to the random number table.Patients in the experiment grouPwere treated with PPH in combination with needle warming moxibustion and Yichangtongmi decoction，while patients in the control grouPwere treated with PPH combined with Lactulose Oral Solution.The defecation，clinical efficacy and relapse rate 1 year after treatment were observed and compared between the two groups.Results After 1 month of treatment，the scoring of defecation time，defecation frequency，feeling of incomplete defecation and defecation smoothness were respectively（1.30±0.96）points，（1.35±0.95）points，（1.53±0.85）points and（1.40±0.93）points in the experiment group，which all showed statistically significant differences as compared with the corresponding（1.89±0.87）points，（2.05±1.10）points，（1.95±0.88）points and（1.84±0.99）points in the control group，all P<0.05.After 1 month of treatment，the total effective rates were 95.75% and 78.38% respectively in the experiment grouPand the control group，between which the comparison showed statistically significant difference（P<0.05）；One year after treatment，among the 33 cured patients in the experiment grouPone patient suffered from relapse，while there were 5 patients who relapsed among the 20 cured patients in the control group，and the comparison showed statistically significant difference（P<0.05）.Conclusion PPH combined with needle warming moxibustion and Yichangtongmi decoction in the treatment of outlet obstructive constipation in the elderly can significantly alleviate the constipation symptoms of patients and reduce the recurrence rate，deserving to be promoted in clinical practice. |
