          【摘要】 目的 探究烧伤创疡再生医疗技术(moist exposed burn therapy/moist exposed burn ointment, MEBT/MEBO) 联合自体纳米脂肪治疗慢性难愈合创面的临床疗效?方法 选取2016 年6 月至2018 年8 月南通市第三人民医院?南通市第四人民医院及扬州瑞和护理康复医院收治的30 例慢性难愈合创面患者作为研究对象, 每例患者选择2 处相似创面或同一创面均分后按照随机数表法随机分为试验组(30 处创面) 与对照组(30 处创面)进行随机自身对照研究, 其中对照组创面采用MEBT/ MEBO 治疗, 试验组创面在对照组治疗的基础上加用自体纳米脂肪治疗, 对比观察两组创面的愈合效果及愈合时间?结果 治疗过程中, 因故退出者3 例, 因依从性较差而影响方案实施或数据采集者6 例, 最终按照课题设计要求完成试验者21 例?治疗4 周后, 21 例患者中试验组创面总有效率为95.2%?愈合率为(88.5±12.2)%, 明显优于对照组创面的总有效率61.9%?愈合率(56.3±21.2)%, P 均<0.01, 差异具有统计学意义; 治疗8 周后, 21 例患者中试验组创面显效率为95.2%?愈合率为(98.1±1.8)%, 明显优于对照组创面的显效率66.7%?愈合率(85.9±15.3)%, P 均<0.05, 差异具有统计学意义; 最终21 例患者创面均完全愈合, 其中试验组创面愈合时间为(36.7±16.2) d, 明显优于对照组的创面愈合时间(62.4±25.2) d, P <0.01, 差异具有统计学意义; 出院后随访2 个月, 所有患者愈后皮肤均无明显瘢痕增生, 溃疡无复发?结论 MEBT/MEBO 联合自体纳米脂肪治疗慢性难愈合创面, 可明显改善创面愈合能力, 提高创面愈合效果, 缩短创面愈合时间, 疗效显著, 且操作简便, 是一种比较理想的慢性难愈合创面的治疗方法?

          【Abstract】Objective To explore the clinical effect of MEBT/MEBO combined with autologous nano-fat in the treatment of chronic hard-to-healing wounds. Methods Thirty patients with chronic hard-to-healing wounds, admitted into Nantong Third People’s Hospital, Nantong Fourth People’s Hospital and Yangzhou Ruihe Nursing and Rehabilitation Hospital from June 2016 to August 2018, were included as subjects. Two similar wounds (or one wound was evenly split into two) in each patient were divided, according to the random number table, into an experiment group (30 wounds) and a control group (30 wounds). The wounds in the control group were treated with MEBT/MEBO whereas the wounds in the experimental group were treated with MEBT/MEBO plus autologous nano-fat. Healing effect and healing time were compared between the two groups. Results During the course of treatment, 3 patients gave up treatment for some reasons, treatment or data collection was failed in 6 patients due to their poor compliance, and thus 21 patients completed the whole study according to the study design. After 4 weeks of treatment, of the 21 patients in the experiment group, the total effective rate was 95.2% and the healing rate was (88.5±12.2) %, which were significantly higher than the corresponding 61.9% and (56.3±21.2) % in the control group, both P < 0.01. After 8 weeks of treatment, of the 21 patients in the experiment group, the effective rate was 95.2% and the healing rate was (98.1±1.8) %, and statistically significant differences were observed as compared with the corresponding 66.7% and (85.9±15.3) % in the control group (both P < 0.05). Eventually, the wounds of the 21 patients were completely healed. The wound healing time in the experiment group was (36.7±16.2) d, which was significantly shorter than (62.4±25.2) d in the control group (P < 0.01). During the follow-up 2 months after discharge, no obvious hypertrophic scar or relapse of ulcers was observed on all healed skin. Conclusion MEBT/MEBO combined with autologous nano-fat in the treatment of chronic hard-to-healing wounds can achieve remarkable curative effects such as significantly improving wound healing ability, promoting healing effect and shortening wound healing time and etc. In addition to its simple and easy operation, this method should be regarded an ideal way of treating chronic hard-to-healing wounds.
