【摘要】 目的 分析探讨高频超声对膝关节半月板急慢性损伤的诊断价值?方法 选取2017 年1 月至2018 年2 月牡丹江医学院第二附属医院收治的50 例膝关节半月板急慢性损伤患者(设为研究组) 及50 例健康体检者(设为对照组) 作为研究对象, 应用高频超声检测其膝关节滑膜厚度?髌上囊积液深度及滑膜血流状况,并予以对比?结果 研究组患者膝关节半月板在基线外侧较为突出且可见点状或条状强回声区, 内侧副韧带变形, 部分患者伴有不同程度的水肿, 滑膜厚度为(6.25±2.20) mm, 髌上囊积液深度为(7.35±1.45) mm, 滑膜血流为0 级者23 例?Ⅰ级者12 例?Ⅱ级者12 例?Ⅲ级者3 例; 对照组研究对象膝关节半月板回声强度中等且均匀, 边缘清晰, 滑膜厚度为(1.10±0.46) mm, 髌上囊积液深度为(2.12±0.43) mm, 滑膜血流为0 级者38例?Ⅰ级者12 例?两组研究对象膝关节滑膜厚度?髌上囊积液深度及滑膜血流状况对比, P 均< 0.01, 差异具有统计学意义?结论 高频超声对膝关节滑膜厚度?髌上囊积液及滑膜血流均较敏感, 在膝关节半月板急慢性损伤的诊断中特异性较高, 且操作简便, 可作为膝关节半月板急慢性损伤的首选诊断方法予以推广?普及? |

【Abstract】 Objective To analyze the diagnostic value of high frequency ultrasound in acute and chronic injuries of knee joint meniscus. Methods A total of admitted 50 patients with acute and chronic injuries of knee joint meniscus (study group) and 50 healthy persons (control group) underwent physical checkup in The Second Affiliated Hospital of Mudanjiang Medical University from January 2017 to February 2018, were chosen as subjects. High frequency ultrasound was used to detect the thickness of their knee joint synovium, the depth of effusion in their suprapatellar bursa and the blood flow condition of synovium, and the results were compared. Results In the study group, the knee joint meniscus protruded from the outside of its baseline, with intense dot or strip echo zones observable, the medial collateral ligament was deformed, and some patients suffered from edema of different degrees. The thickness of synovium was (6.25±2.20) mm, the depth of effusion in suprapatellar bursa was (7.35±1.45) mm, and 23 cases showed grade 0 blood flow status of synovium, 12 cases grade I, 12 cases grade II and 3 cases grade III. In contrast, in the control group, the echo intensity of knee joint meniscus was moderate and uniform and the margins of meniscus were well defined. The thickness of knee joint synovium was (1.10±0.46) mm, the depth of effusion in suprapatellar bursa was (2.12±0.43) mm, and 38 cases showed grade 0 blood flow status of synovium and 12 cases grade I. The thickness of knee joint synovium, the depth of effusion in suprapatellar bursa and synovium blood flow condition were respectively compared between the two groups, which all showed statistically significant differences (P < 0.01). Conclusion High frequency ultrasound is a very sensitive way of checking the thickness of knee joint synovium, effusion in suprapatellar bursa and synovium blood flow, with high specificity in the diagnosis of acute and chronic injuries of knee joint meniscus. This method is also easy to operate, it can be promoted as a preferred diagnostic approach for acute and chronic injuries of knee joint meniscus. |
