【摘要】 目的 分析探讨湿润烧伤膏(moist exposed burn ointment,MEBO) 药纱治疗Wanger 3级糖尿病足的临床疗效?方法 选取2015年3月至2016年11月台山市中医院外科收治的60 例Wanger 3级糖尿病足患者作为研究对象,并按照随机数表法将其随机分为试验组(30 例) 和对照组(30 例),其中试验组患者采用MEBO药纱治疗,对照组患者采用凡士林油纱治疗,对比观察两组患者自受试之日起至创面愈合的换药次数、每次换药所需时间、换药费用、创面愈合时间、揭除敷料时的疼痛程度及临床疗效等?结果 试验组患者的换药次数为(45.5±9.2) 次、换药时间为(11.1±1.4) min、换药费用为(1255.36±171.65) 元、创面愈合时间为(43.2±5.5) d,均明显优于对照组患者的换药次数(59.7 ± 10.5) 次、换药时间(17.7±2.1) min、换药费用(1693.60±209.94) 元、创面愈合时间(52.5±7.4) d,P 均<0.01,差异具有统计学意义;治疗第8 天,试验组患者揭除敷料时创面无疼痛者17 例、轻度疼痛者9 例、中度疼痛者3 例、重度疼痛者1 例,明显优于对照组的轻度疼痛者6 例、中度疼痛者15 例、重度疼痛者9 例,P<0.01,差异具有统计学意义;治疗6 周后,试验组患者中治愈24 例、好转5 例、无效1 例,明显优于对照组的治愈12 例、好转9 例、无效9 例,P <0.01,差异具有统计学意义?结论 MEBO 药纱可有效促进Wanger 3级糖尿病足创面的愈合,缩短创面愈合时间,减少换药次数及换药时间,降低揭除敷料时的疼痛程度及换药费用,疗效显著? |

【Abstract】Objective: To observe the clinical efficacy of Moist Exposed Burn Ointment (MEBO) impregnated gauze in the treatment of diabetic foot of Wagner grade III. Methods: The subjects were sixty patients with diabetic foot of Wagner grade III admitted into the Surgery Department of Taishan TCM Hospital between March 2015 and November 2016. According to the random number table, the 60 subjects were divided into an experiment group (30 patients) and a control group (30 patients), and patients in the experiment group were treated with MEBO-impregnated gauze while patients in the control group was given Vaseline gauze. From the starting date of the study, the number of dressing change till wound healing, time used for each dressing change, cost of dressing change, wound healing time, pain degree when removing the dressing and clinical efficacy were observed and compared between the two groups. Results: In the experiment group, the number of dressing change were (45.5±9.2) times, time used for each dressing change (11.1±1.4)min, cost of dressing change (1255.36±171.65) RMB and wound healing time (43.2±5.5)d, which were all obviously superior to the corresponding (59.7±10.5) times, (17.7±2.1)min, (1693.60±209.94) RMB and (52.5±7.4)d in the control group, and the differences were all statistically significant, P <0.01. On day 8 of treatment, when the dressing was removed in the experiment group, 17 patients felt no pain, 9 patients felt mild pain, 3 patients moderate pain and one patient severe pain, which were also markedly better than mild pain in 6 patients, moderate pain in 15 patients and severe pain in 9 patients in the control group, and the differences were statistically significant, P<0.01. After 6 weeks of treatment, in the experiment group 24 patients were cured, 5 patients got improvement and only one patient showed ineffectiveness, while in the control group 12 patients were cured, 9 patients got improvement and 9 patients showed ineffectiveness, between which the comparison showed statistically significant difference, P<0.01. Conclusion: MEBO-impregnated gauze can achieve significant clinical efficacy in the treatment of diabetic foot of Wagner grade III, including effectively promoting the wound healing, shortening wound healing time, reducing the number of dressing change and time used for dressing change, alleviating patients’ pain when removing the dressing, and decreasing the cost of dressing change. |
