【摘要】 目的 分析探讨湿润烧伤膏预防气压止血带致皮肤损伤的临床疗效。方法 按照随机数表法将2016年7月至2017年7月南京医科大学附属南京邦德骨科医院与丹阳市人民医院收治的60例四肢手术后需使用气压止血带治疗的患者随机分为试验组(30例)与对照组(30例),其中试验组患者于常规缚扎止血带前在缚扎止血带部位均匀涂抹湿润烧伤膏,对照组患者仅按常规方法缚扎止血带,对比观察两组患者的手术时间、止血效果及皮肤损伤情况。结果 试验组患者的手术时(2.5±1.3)h,止血效果为优者28例、良者2例;对照组患者的手术时间为(2.7±1.4)h,止血效果为优者27例、良者3例,两组对比,P均>0.05,差异无统计学意义。解除止血带后即刻,试验组患者的皮肤损伤总发生率为20.0%,对照组患者的皮肤损伤总发生率为86.7%,两组对比,P<0.01,差异具有统计学意义。结论 缚扎止血带部位均匀涂抹湿润烧伤膏,可在保证止血效果的同时,有效减少气压止血带所致的皮肤损伤。 |

【Abstract】Objective To investigate the therapeutic effect of MEBO in preventing skin damage caused by pneumatic tourniquet. Methods Sixty patients with the necessity of using pneumatic tourniquet after limb surgery in Bond Orthopaedic Hospital of Nanjing Medical University and Danyang People’s Hospital from July 2016 to July 2017 were divided, according to the random number table, into an experiment group (30 cases) and a control group (30 cases). For patients in the experiment group, a layer of MEBO was smeared onto the location of wearing the tourniquet before wearing it, while in the control group only the routine wearing of the tourniquet was given. The operation time, hemostatic effect and skin damage condition were compared between the two groups. Results In the experiment group, the operation time was (2.5±1.3)h, excellent hemostatic effect was observed in 28 cases and good hemostatic effect in 2 cases, which showed no statistically significant differences as compared with the operation time (2.7±1.4)h, excellent hemostatic effect in 27 cases and good in 3 cases in the control group (both P>0.05). Immediately after the tourniquet was removed, the total incidences of skin damage were 20.0% and 86.7% respectively in the experiment group and the control group, showing a statistically significant difference (P<0.01). Conclusion The application of MEBO at the location of wearing the tourniquet, apart from its definite hemostatic effect, can effectively reduce skin damage caused by pneumatic tourniquet. |
