【摘要】 目的 探讨湿润烧伤膏 ( moist exposed burn ointment, MEBO) 外敷联合红外线照射对寻常型天疱疮患者部分血清细胞因子表达水平的影响? 方法 将 2013 年 1 月至 2018 年 6 月广州医科大学附属第五医院及南方医科大学皮肤病医院收治的 44 例寻常型天疱疮患者随机分为观察组 (22 例患者, 261 处创面) 和对照组 (22 例患者, 253 处创面), 其中观察组患者皮损创面采用 MEBO 外敷联合红外线照射治疗, 对照组患者皮损创面采用依沙吖啶溶液外敷联合红外线照射治疗, 对比观察两组患者治疗第 14 天的临床疗效和血清 IL?6? IL?10?IL?17? IL?23? IFN?γ? TNF?α 表达水平的变化情况及住院时间? 结果 治疗第 14 天, 观察组患者的治疗有效率为 81 23% , 明显优于对照组患者的治疗有效率 57 31% , 两组对比, P < 0 05, 差异具有统计学意义; 治疗第 14 天, 两组患者血清 IL?10 及 TNF?α 表达水平对比, P 均 > 0 05, 差异无统计学意义, 血清 IL?6? IL?17? IL?23 及IFN?γ表达水平对比, P 均 < 0 05, 差异具有统计学意义; 观察组患者的住院时间为 (21 9 ± 7 8) d, 明显短于对照组患者的住院时间 (32 8 ± 11 5) d, 两组对比, P < 0 05, 差异具有统计学意义? 结论 MEBO 外敷联合红外线照射可缩短寻常型天疱疮患者的创面愈合时间, 提高创面愈合率, 疗效显著, 可能与其能够调节天疱疮患者血清 IL?6? IL?17? IL?23 及 IFN?γ 等细胞因子的表达水平有关, 值得临床进一步推广应用与验证? |

【Abstract】 Objective To investigate the influence of external application of MEBO combined with infrared irradi? ation on the expression level of serum cytokines in patients with pemphigus vulgaris. Methods Forty?four patients with pemphigus vulgaris, admitted into The Fifth Affiliated Hospital of Guangzhou Medical University and Dermatology Hospital of Southern Medical University from January 2013 to June 2018, were randomly divided into an observation group (22 pa? tients, 261 wounds) and a control group (22 patients, 253 wounds). The wounds of patients in the observation group were treated with external application of MEBO combined with infrared irradiation whereas the wounds of patients in the control group with external application of ethacridine lactate solution combined with infrared irradiation. The clinical efficacy, the expression changes of serum IL?6, IL?10, IL?17, IL?23, IFN?γ and TNF?α on day 14 of treatment, and hospitalization time were observed and compared between the two groups. Results On day 14 of treatment, the effective rate in the observation group was 81 23% , much higher than 57 31% in the control group, and the comparison of the effective rate between the two groups showed statistically significant difference ( P < 0 05). On day 14 of treatment, the expression levels of serum IL?10 and TNF?α were respectively compared between the two groups, which showed no statistically significant difference( both P > 0 05), whereas the expression levels of serum IL?6, IL?17, IL?23 and IFN?γ were respectively compared be? tween the two groups and the results all showed statistically significant differences ( P < 0 05); The hospitalization time in the observation group was (21 9 ± 7 8) d, much shorter than (32 8 ± 11 5) d in the control group, and the difference showed statistically significance ( P < 0 05). Conclusion In the treatment of pemphigus vulgaris, the external application of MEBO combined with infrared irradiation can achieve remarkable clinical efficacy such as shortening the wound healing time and improving wound healing rate, which might be associated with MEBO’ s effect in regulating the expression levels of serum IL?6, IL?17, IL?23 and IFN?γ, and the treatment modality deserves being further applied and validated. |
