          【摘要】 目的 探讨 300 mmol/L (mM)高渗盐溶液 (HS) 对重度烫伤大鼠肾损伤的影响? 方法 按照随 机数表法将 56 只大鼠随机分为假伤组 (8 只)? LR 组 (24 只) 与 HS 组 (24 只),其中假伤组大鼠做假伤处理后直接处死,LR 组大鼠建立重度烫伤模型后予以乳酸钠林格注射液 (LR) 复苏治疗,HS 组大鼠建立重度烫伤模型后予以 300 mM HS (1 L 300 mM HS 含 97 mL 10% NaCl 和 903 mL LR) 复苏治疗,对比观察各组大鼠肾脏组织湿干重比值 (W/D)? p38 丝裂原活化蛋白激酶 (p38 mitogen-activated protein kinases,p38 MAPK) 通道活化程度及血清肌酐 (Cr)? 中性粒细胞明胶酶相关脂质运载蛋白 (neutrophil gelatinase-associated lipocalin,NGAL)? 肿瘤坏死因子-α (TNF-α)? 白细胞介素-6 (IL-6)? 高迁移率族蛋白 B1 (highmobility group box1,HMGB1) 的水平 变化? 结果 与假伤组对比,LR 组及 HS 组大鼠伤后2? 8? 24 h 肾脏组织 W/D? p38 MAPK 通道活化程度及血清 Cr? NGAL? TNF-α? IL-6 水平,以及伤后 8? 24 h HMGB1 水平均较高,P 均 < 0.05,差异具有统计学意义;而伤 后 2 h HMGB1 水平与假伤组相同,P > 0.05,差异无统计学意义? 与 LR 组对比,HS 组大鼠伤后 2? 8? 24 h 肾脏 组织 W/D? p38 MAPK 通道活化程度均较低,P 均 < 0.05,差异具有统计学意义;HS 组大鼠伤后 2 h 血清 Cr?           【Abstract】 Objective To explore the influence of 300 mmol/L (mM) hypertonic saline (HS) on the renal injury of severely scalded rats. Methods Fifty-six rats were randomly divided into a sham group (8 rats),LR group (24 rats) and HS group (24 rats) according to the random number table. In the sham group,rats were killed directly after fake injury management,rats in the LR group were injected sodium lactate ringer’s injection (LR) for resuscitation after the establish- ment of severe scalds,while rats in the HS group were given 300 mM HS (1 L of 300 mM HS containing 97 mL 10% NaCl and 903 mL LR) for resuscitation after the establishment of severe scalds. In the three groups,the wet/dry weight ratio (W/D) of renal tissues,the activation degree of p38 mitogen-activated protein kinases (p38 MAPK) channel,and the expression levels of serum creatinine (Cr),neutrophil gelatinase-associated lipocalin (NGAL),tumor necrosis factor-α (TNF-α),interleukin-6 ( IL-6) and high mobility group box 1 (HMGB1 ) were respectively observed and compared. Results Compared with the sham group,the W/ D,activation degree of p38 MAPK channel and the expression levels of serum Cr,NGAL,TNF-α and IL-6 at 2 h,8 h and 24 h after scalds,and the expression levels of HMGB1 at 8 h and 24 h after scalds in the LR group and HS group were all improved,and the differences all showed statistical significance (all P < 0.05),while the expression level of HMGB1 at 2 h after scalds was same as that in the sham group,presenting no statistically significant difference,P > 0.05. The W/D and the activation degree of p38 MAPK channel in renal tissues in the HS group at 2 h,8 h and 24 h after scalds were lower as compared with that in the LR group,and the differences were statistically significant,all P < 0.05. In the HS group,the expression levels of serum Cr and HMGB1 at 2 h after scalds were basically the same as that in the LR group,and the differences showed no statistical significance (P > 0.05),while the expression levels of serum Cr and HMGB1 at 8 h and 24 h after scalds were lower than that in the LR group,and the differences showed statistical significance,all P < 0.05. The expression levels of serum NGAL,TNF-α and IL-6 in the HS group at 2 h,8 h and 24 h after scalds were all lower than that in the LR group,and the differences showed statistical significance,all P < 0.05. Conclusion HS can alleviate the renal damage of severely scalded patients by inhibiting the production of serum inflammatory mediators and downregulating the activation of p38 MAPK channel,and thus it can be applied at the early stage of burns to fight shock.
