【摘要】 目的 初步评估不同温度的湿润烧伤膏 (moist exposed burn ointment, MEBO) 对大鼠超脉冲 CO2 点阵激光术后皮肤屏障功能的影响? 方法 按照随机数表法将 30 只 Wistar 大鼠随机分为空白对照组? 正常对照 组? 冷敷组? MEBO 低温组? MEBO 常温组, 每组 6 只? 空白对照组大鼠仅做背部备皮处理, 正常对照组? 冷敷 组? MEBO 低温组及 MEBO 常温组大鼠均建立超脉冲 CO2 点阵激光术后创面模型, 建模成功后正常对照组大鼠创 面不做任何处理; 冷敷组大鼠创面予以 4 ℃水袋外敷 30 min, 每天 1 次; MEBO 低温组大鼠创面予以 4 ℃ MEBO 外涂, 每天换药 1 次; MEBO 常温组大鼠创面予以 28 ℃ MEBO 外涂, 每天换药 1 次? 对比观察激光术前及激光 术后即刻? 12 h? 24 h? 7 d 各组大鼠皮肤或创面经皮水分丢失 ( transepidermal water loss, TEWL) 值? 结果 激 光术后, 正常对照组? 冷敷组? MEBO 低温组? MEBO 常温组大鼠创面 TEWL 值均呈先升高后降低的趋势 (F = 66.740? 106.100? 91.080? 70.860, P 均 = 0.000, P 均 < 0.05), 至激光术后 7 d, 正常对照组与 MEBO 常温组 大鼠创面 TEWL 值均恢复至与术前相当的水平 (P 均 > 0.05)? 激光术后即刻? 12 h? 24 h, 正常对照组? 冷敷 组? MEBO 低温组? 【Abstract】 Objective To tentatively evaluate the effect of MEBO at different temperatures on the skin barrier function of rats after the ultrapulsed CO2 fractional laser operation. Methods Thirty Wistar rats were divided, according to the random number table, into blank control group, normal control group, cold compress group, MEBO low temperaturegroup and MEBO room temperature group, with 6 rats in each group. For rats in the blank control group, only back skin preparation was performed, while the wound models of the ultra pulsed CO2 fractional laser operation were established respectively in the normal control group, cold compress group, MEBO low temperature group and MEBO room temperature group. After the modelling, the wounds in the normal control group were left untreated; the wounds in the cold compress group were managed with water bags of 4 ℃ for 30 minutes, once per day; the wounds in the MEBO low temperature group were managed with the external application of MEBO of 4 ℃ , and the dressing was changed once per day; and in the MEBO room temperature group the wounds were given the external application of MEBO of 28 ℃ , and also the dressing change was performed once per day. The transepidermal water loss (TEWL) values of the wounds or skin in each group were observed and respectively compared at the time points of before the laser operation, immediately after the laser operation, 12 h, 24 h and day 7 after the laser operation. Results After the laser operation, the wound TEWL values in the normal control group, cold compress group, MEBO low temperature group and MEBO room temperature group increased first and then decreased (F = 66.740, 106.100, 91.080, 70.860, all P = 0.000, all P < 0.05). On day 7 after the laser operation, the skin or wound TEWL values in the normal control group and MEBO room temperature group almost returned to their preoperative levels (both P > 0.05). At the time points of immediately after the laser operation, 12 h and 24 h after the laser oper? ation, the skin or wound TEWL values in the normal control group, cold compress group, MEBO low temperature group and MEBO room temperature group were significantly higher than that in the blank control group (all P < 0.05), and the wound TEWL values in the cold compress group and MEBO low temperature group were much higher than that in the normal control group (both P < 0.05). On day 7 after the laser operation, the wound TEWL value in the MEBO room temperature group basically returned to the level in the blank control group (P > 0.05) and was significantly lower than that in the normal control group, cold compress group and MEBO low temperature group (all P < 0.05). In contrast, the wound TEWL values in all the other four groups were significantly higher than that in the blank control group ( all P < 0.05). Conclusion Impaired skin barrier function will appear in the postoperative wound of the ultra pulsed CO2 fractional laser operation, and MEBO at the room temperature (28 ℃ ) can accelerate the wound healing and promote the recovery of skin barrier function. |
