【摘要】 目的 对比观察悬浮床联合负压封闭辅助伤口闭合技术治疗烧伤复合软组织损伤患者难愈性创面的临床疗效?方法 选取2015 年10 月至2018 年10 月中国人民解放军联勤保障部队第九八八医院收治的84 例烧伤复合软组织损伤致难愈性创面患者作为研究对象, 并按照随机数表法将其随机分为研究组(42 例) 与对照组(42 例), 其中研究组患者采用悬浮床联合负压封闭辅助伤口闭合技术治疗局部创面, 对照组患者单纯采用负压封闭辅助伤口闭合技术治疗局部创面, 对比观察两组患者治疗前及治疗第7?14 天创面分泌物细菌培养结果?细菌转阴率?白细胞计数?C 反应蛋白水平?红细胞沉降率与疼痛评分, 以及感染控制时间与创面愈合时间?结果 治疗第7 天, 研究组患者的细菌转阴率为55.88%, 明显高于对照组患者的细菌转阴率30.56% (χ2 =4.580,P =0.032); 治疗第14 天, 研究组患者的细菌转阴率为85.29%, 与对照组患者的细菌转阴率72.22% 相近(χ2 =1.775, P =0.183)?治疗第7 天, 研究组患者的白细胞计数?C 反应蛋白水平?红细胞沉降率及疼痛评分均明显低于对照组(t = 2.570?2.424?2.384?3.207, P = 0.012?0.018?0.019?0.002); 治疗第14 天, 研究组患者的白细胞计数?C 反应蛋白水平?红细胞沉降率及疼痛评分亦明显低于对照(t=2.269?4.153?2.597?2.397,P=0.026?0.000?0.011?0.019)?研究组患者感染控制时间及创面愈合时间均明显短于对照组(t =2.014?2.535, P =0.047?0.013)?结论 悬浮床联合负压封闭辅助伤口闭合技术可有效减轻烧伤复合软组织损伤患者的炎症反应程度, 控制创面感染, 促进创面愈合, 疗效显著, 临床应用价值较高? |

【Abstract】 Objective To compare the therapeutic effect of suspended bed combined with vacuum-assisted closure technique in the treatment of refractory wounds in patients with burn and soft tissue injury. Methods Eighty-four patients with refractory wounds caused by burn combined soft tissue injury, who were admitted to the 988 Hospital of PLA Joint Service Support Force from October 2015 to October 2018 were divided, according to the random number table, into a study group (42 cases) and a control group (42 cases). The patients in the study group were treated with suspended bed combined with vacuum-assisted closure technique while the patients in the control group were treated simply with vacuum-assisted closure technique. The following indicators including bacterial culture results, bacterial negative conversion rate, white blood cell count, C-reactive protein level, erythrocyte sedimentation rate of the wound secretion, and pain score before the treatment and on day 7 and 14 of treatment, as well as infection control time and wound healing time were observed and compared between the two groups. Results On day 7 of treatment, the bacterial negative conversion rate in the study group was 55.88%, which was significantly higher than 30.56% in the control group (χ2 = 4.580, P = 0.032); On day 14 of treatment, the bacterial negative conversion rate in the study group was 85.29%, which was similar to 72.22% in the control group (χ2 =1.775, P =0.183). On day 7 of treatment, the white blood cell count, C-reactive protein level, erythrocyte sedimentation rate and pain score in the study group were all significantly lower than that in the control group (t =2.570, 2.424, 2.384 and 3.207 respectively, P =0.012, 0.018, 0.019 and 0.002 respectively), and on day 14 of treatment, the white blood cell count, C-reactive protein level, erythrocyte sedimentation rate and pain score in the study group were also significantly lower than that in the control group (t =2.269, 4.153, 2.597 and 2.397 respectively, P =0.026,0.000, 0.011 and 0.019 respectively). The wound infection control time and wound healing time in the study group were both significantly shorter than that in the control group (t =2??014 and 2??535 respectively, P =0??047 and 0??013 respectively).Conclusion Suspended bed combined with vacuum-assisted closure technique can effectively reduce the degree of inflammatory reaction in patients with burn and soft tissue injury, control wound infection and promote wound healing, presenting a higher clinical application value. |
