          【摘要】 目的 探究微型钢板内固定治疗后踝大块骨折的临床疗效和对踝关节功能的影响?方法 将2017年11 月至2018 年12 月郑州市第七人民医院收治的60 例后踝大块骨折患者随机分为观察组(30 例) 与对照组(30 例), 其中观察组患者采用微型钢板内固定治疗, 对照组患者采用空心钉内固定治疗?对比观察两组患者后踝固定效果?手术时间?骨折愈合时间?住院时间及踝关节功能评分?结果 观察组患者后踝固定优良率为83.33%, 明显高于对照组患者的后踝固定优良率66.67%, 两组对比, P <0.05, 差异具有统计学意义; 观察组患者的手术时间为(62.13 ±10.75) min, 骨折愈合时间为(8.10 ±1.42) 周, 明显短于对照组患者的手术时间(78.45 ±10.03) min?骨折愈合时间(9.74 ±1.61) 周, 两组对比, P 均< 0.05, 差异具有统计学意义; 术后6个月, 观察组患者AOFAS 评分为(98.13 ±8.75) 分, 明显高于对照组患者的AOFAS 评分(80.45 ± 8.03) 分,两组对比, P <0.05, 差异具有统计学意义?结论 微型钢板内固定能够显著提高后踝大块骨折患者的后踝固定效果, 缩短手术时间及骨折愈合时间, 增强踝关节功能, 效果显著, 临床应用价值较高?

          【Abstract】 Objective To investigate the clinical efficacy of the internal fixation with miniplate in treating large posterior malleolar fracture and its influence on the ankle joint functions. Methods Sixty patients with large posterior malleolar fracture admitted into The Seventh People’s Hospital of Zhengzhou from November 2017 to December 2018 were randomly divided into an observation group (30 patients) and a control group (30 patients). In the observation group the patients were treated with the internal fixation with miniplate while the patients in the control group were treated with the cannulated screw internal fixation. The following indicators including posterior malleolar fixation effect, operative time, fracture healing time, hospital stay and the function score of ankle joint were observed and compared between the two groups.Results The good rate of the posterior malleolar fixation effect in the observation group was 83.33%, which was obviously higher than the 66.67% in the control group, and the difference was statistically significant (P <0.05). The operative time and fracture healing time in the observation group were respectively (62.13±10.75) min and (8.10±1.42) week, which were both obviously shorter than the corresponding (78.45 ± 10.03) min and (9.74 ± 1.61) week in the control group,and the differences were also statistically significant (both P <0.05). On month 6 postoperation, the AOFAS score in the observation group was (98.13±8.75) point, which presented a statistically significant difference as compared with the(80.45±8.03) point in the control group (P <0.05). Conclusion The internal fixation with miniplate in the treatment of large posterior malleolar fracture can significantly improve the fixation effect of the posterior malleolar, shorten the operative time and fracture healing time, and enhance the ankle joint functions, presenting a higher clinical application value.
