| 【摘要】目的 对比观察结肠水疗联合益肠通秘汤及乳酸菌素片治疗脾肾阳虚型功能性便秘的临床疗效。方法 将2017年1月至2018年1月湖北省中医院肛肠科收治的91例脾肾阳虚型功能性便秘患者按照随机数表法分为治疗组（46例）与对照组（45例），其中治疗组患者采用结肠水疗联合益肠通秘汤及乳酸菌素片治疗，对照组患者采用结肠水疗联合枸橼酸莫沙必利片治疗，对比观察两组患者治疗30 d时的排便情况、临床疗效及治疗1年后的复发情况。结果 治疗30 d时，治疗组患者排便耗时、排便频率、排便顺畅度及便后不尽感评分明显低于对照组（t=4.487、4.734、4.647、4.262，P=0.003、0.004、0.004、0.008），差异具有统计学意义；治疗30 d时，治疗组患者治疗总有效率为95.65%，明显高于对照组患者的治疗总有效率77.78%（Mann-Whitney U=766.000，Z=-2.396，P=0.017），差异具有统计学意义。治疗1年后随访，治疗组32例痊愈患者中复发2例，复发率为6.25%；对照组21例痊愈患者中复发6例，复发率为28.57%。结论 与结肠水疗联合枸橼酸莫沙必利片相比，结肠水疗联合益肠通秘汤及乳酸菌素片更能有效改善脾肾阳虚型功能性便秘患者的便秘症状，且复发率较低，疗效显著，值得临床推广应用。 |

| 【Abstract】Objective To observe and compare the clinical efficacy of colon hydrotherapy in combination with Yichang Tongmi Decoction and lactobacillus tablets in the treatment of functional constipation of spleen-kidney yang deficiency type. Methods Ninety-one patients with functional constipation of spleen-kidney yang deficiency type, admitted to Anorectal Department of Hubei Provincial Hospital of Traditional Chinese Medicine from January 2017 to January 2018, were divided, according to the random number table, into a treatment group (46 cases) and a control group (45 cases). In the treatment group, the patients were treated with colon hydrotherapy in combination with Yichang Tongmi Decoction and lactobacillus tablets while patients in the control group were treated with colon hydrotherapy in combination with Mosapride citrate tablets. Indicators such as defecation and clinical efficacy on day 30 of treatment, and one-year relapse condition were observed and compared between the two groups. Results After 30 days of treatment, the defecation time, defecation frequency, defecation smoothness, and feeling of incomplete defecation in the treatment group were significantly lower than that in the control group (t = 4.487, 4.734, 4.647, 4.262, P = 0.003, 0.004, 0.004, 0.008) and the results of the comparison showed statistically significant difference; the total effective rate of the treatment group was 95.65%, significantly higher than 77.78% of the control group (Mann-Whitney U = 766.000, Z = -2.396 , P = 0.017) and the results of the comparison showed statistically significant difference. During one-year follow-up, 2 of the 32 cured patients in the treatment group relapsed, with a recurrence rate of 6.25%; 6 of the 21 cured patients in the control group relapsed, with a recurrence rate of 28.57%. Conclusion Compared with colon hydrotherapy in combination with Mosapride citrate, colon hydrotherapy in combination with Yichang Tongmi Decoction and lactobacillus tablets can improve the symptom of constipation for patients with functional constipation of spleen-kidney yang deficiency type and the recurrence rate is low. It deserves to be promoted in clinical practice due to its good efficacy. |
