目的 探讨重组牛碱性成纤维细胞生长因子 ( recombinant bovine basic fibroblast growth factor, rb?bFGF)? 湿润烧伤膏 (moist exposed burn ointment, MEBO)? 红/蓝光照射联合应用 (简称三联疗法) 治疗儿童全层皮肤缺损的临床效果? 方法 回顾性总结 2010年3月至2018年10月北大医疗鲁中医院烧伤与创面修复中心收治的采用三联疗法治疗的 35 例全层皮肤缺损患儿的病历资料, 分析创面愈合时间及愈合效果, 探讨三联疗法对儿童全层皮肤缺损的治疗效果? 结果 35 例患儿中, 除 1例放射性损伤患儿创面治疗 20 d 时仍无明显皮岛生成, 予以植皮封闭后愈合外, 其余患儿创面均于三联疗法治疗后愈合? 患儿创面愈合时间为27~69 d [(36.4±6.1) d], 创面愈合后, 2例患儿足部出现瘢痕增生, 予以硅酮霜联合加压包扎治疗后, 未遗留明显功能障碍, 效果满意; 1例患儿出现指蹼粘连, 予以手术松解后, 效果满意; 其余患儿愈后皮肤均无明显瘢痕增生? 结论 三联疗法可有效促进儿童全层皮肤缺损创面愈合, 减少瘢痕增生, 疗效显著, 临床应用价值较高? |

Objective To explore the clinical effect of the combined application of recombinant bovine basic fibroblast growth factor (rb-bFGF), moist exposed burn ointment (MEBO), and red / blue light irradiation (referred to as triple therapy) in treating full-thickness skin defect in children. Methods Medical data of 35 children patients with full-thickness skin defects treated by the triple therapy, admitted to Burn and Wound Repair Center of PKUCare Luzhong Hospital from March 2010 to October 2018, were retrospectively analyzed. The wound healing time and healing effect were analyzed to explore the therapeutic effect of the triple therapy on full-thickness skin defect. Results Among the 35 patients, one patient with radioactive injury was treated with skin grafting but no obvious skin islands formed after 20 days of treatment, and the wound was healed eventually after the closure with skin grafting. In contrast, the wounds in the other 34 patients were all healed after the treatment of the triple therapy, and the wound healing time was 27 - 69 days [ (36.4±6.1) d]. After wound healing, scar hyperplasia developed in the feet of 2 patients. After being treated with silicone cream combined with the pressure bandaging, the feet recovered well without obvious dysfunction. One patient had finger web adhesion which was cured with surgical release. No obvious scar hyperplasia was observed in the healed skin of the other children patients.Conclusion The triple therapy can effectively promote the healing of full-thickness skin defect in children and reduce scar hyperplasia, presenting very high value of clinical application. |
