          目的 分析探讨负压封闭引流联合湿润烧伤膏序贯治疗骶尾部Ⅲ期压疮的应用效果及部分作用机制。方法 选取2017年8月至2019年8月郑州大学第三附属医院收治的84例骶尾部Ⅲ期压疮患者作为研究对象,并按照随机数表法将其随机分为观察组与对照组,其中观察组患者采用负压封闭引流联合湿润烧伤膏序贯治疗局部创面,对照组患者单纯采用负压封闭引流治疗局部创面,对比两组患者创面愈合时间及基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)与基质金属蛋白酶抑制剂-1(tissue inhibitor of metalloproteinase-1,TIMP-1)的变化情况。结果 治疗第3天,观察组患者MMP-9、TIMP-1表达水平明显高于对照组(t=7.053、5.479,P均=0.000),而治疗第7、14天,观察组患者MMP-9、TIMP-1表达水平均明显低于对照组(第7天:t=7.378、9.438,P均=0.000;第14天:t=22.300、26.790,P均=0.000);观察组患者创面愈合时间明显短于对照组(t=2.652,P=0.010)。结论 负压封闭引流与湿润烧伤膏序贯治疗可有效促进骶尾部Ⅲ期压疮创面的愈合,且调节MMP-9及TIMP-1的表达水平可能是其部分作用机制。

          Objective To analyze the application effect and action mechanism of the sequential treatment approach of VSD (vacuum sealing drainage) and MEBO in treating stage III sacrococcygeal pressure ulcers. Methods Eighty-four patients with stage III sacrococcygeal pressure ulcers, admitted to the Third Affiliated Hospital of Zhengzhou University from August 2017 to August 2019, were selected as research subjects and they were divided, according to the random number table, into an observation group and a control group. For patients in the observation group, the sequential treatment consisting of VSD and MEBO was performed, whereas patients in the control group were treated only with VSD. The wound healing time and changes of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) of patients were analyzed and compared between the two groups. Results On day 3 of treatment, the expression levels of MMP-9 and TIMP-1 in the observation group were significantly higher than those in the control group (t=7.053 and 5.479, both P=0.000). On day 7 and 14 of treatment, the expression levels of MMP-9 and TIMP-1 in the observation group were all significantly lower than those in the control group (day 7: t=7.378 and 9.438, both P=0.000; day 14: t=22.300 and 26.790, both P=0.000); the wound healing time in the observation group was significantly shorter than that in the control group (t=2.652, P=0.010). Conclusion The sequential treatment approach of VSD and MEBO can effectively promote the healing of stage III sacrococcygeal pressure ulcer, of which regulating the expression levels of MMP-9 and TIMP-1 may be part of its action of mechanism.
