目的 分析探讨湿润烧伤膏联合负压封闭引流在慢性难愈合创面植皮术后的治疗效果。方法 选取2015年3月至2017年5月信阳职业技术学院附属医院收治的100例慢性难愈合创面患者作为研究对象,并按照治疗方法将其分为观察组与对照组,每组50例,其中观察组患者于植皮术后行湿润烧伤膏联合负压封闭引流治疗,对照组患者于植皮术后单纯行负压封闭引流治疗,对比两组患者创面愈合时间、瘢痕增生情况以及治疗前后乳酸脱氢酶与琥珀酸脱氢酶水平变化情况。结果 治疗7 d后,观察组患者乳酸脱氢酶水平明显低于对照组(t=22.070,P=0.000),琥珀酸脱氢酶水平明显高于对照组(t=5.240,P=0.000);最终两组患者创面均完全愈合,其中观察组患者创面愈合时间明显短于对照组(t=5.078,P=0.000),且术后6个月愈后皮肤瘢痕评分明显低于对照组(t=12.290,P =0.000)。结论 湿润烧伤膏与负压封闭引流联合应用可有效提高慢性难愈合创面植皮术后的皮片成活率,加快创面愈合,抑制瘢痕增生,且调节局部组织的有氧代谢水平可能是其促进皮片成活及创面愈合的部分作用机制。 |

Objective To analyze and explore the clinical efficacy of MEBO combined with vacuum?sealing?drainage (VSD) on chronic refractory wounds after skin grafting. Methods One hundred patients with chronic refractory wounds, admitted to the Affiliated Hospital of Xinyang Vocational and Technical College from March 2015 to May 2017, were selected as research subjects and were divided, according to their different treatment protocols, into an observation group (50 cases) and a control group (50 cases). Patients in the observation group were treated with MEBO in combination with VSD after skin grafting, whereas patients in the control group were treated with VSD only after skin grafting. The wound healing time, the condition of scar hyperplasia, and changes in levels of lactic dehydrogenase and succinodehydrogenase before and after treatment were compared between the two groups of patients.Results After 7 days of treatment, the level of lactic dehydrogenase in the observation group was significantly lower than that in the control group (t=22.070, P=0.000), and the level of succinodehydrogenase in the observation group was significantly higher than that in the control group (t=5.240, P=0.000). In the end, the wounds in the two groups were all healed. The wound healing time in the observation group was significantly shorter than that in the control group (t=5.078, P=0.000), and 6 months after the skin grafting, the skin scar score in the observation group was significantly lower than that in the control group (t= 12.290, P = 0.000).Conclusion The combined application of MEBO and VSD can effectively improve the survival rate of skin flaps on the postoperative wounds of skin grafting for chronic refractory wounds, speed up the wound healing and inhibit the formation of scar hyperplasia. Regulating aerobic metabolism of local tissues might be a part of the mechanism of action to improve the survival rate of skin flaps and promote wound healing. |
