          目的 探讨早期肠内营养联合益生菌对重型颅脑损伤患者的治疗效果。方法 选取2018年1月至2020年1月开封市中心医院收治的65例重型颅脑损伤患者作为研究对象,并按照随机数表法将其随机分为研究组33例和对照组32例,两组患者在常规治疗的同时,研究组患者行早期肠内营养联合益生菌治疗,对照组患者行早期肠内营养治疗,对比两组患者治疗前后的营养状况、免疫功能及并发症发生情况。结果 治疗14 d时,两组患者血红蛋白(Hb)、血清总蛋白(TP)、白蛋白(ALB)水平均明显升高,且研究组明显高于对照组(t=4.787、3.964、3.589,P均<0.001);治疗14 d时,两组患者血清免疫球蛋白(Ig)A、IgG、IgM水平均明显升高,且研究组明显高于对照组(t=2.784、3.325、2.676,P=0.007、0.002、0.009);研究组患者并发症发生率为6.06%,明显低于对照组患者的并发症发生率28.13%(χ2=5.626,P=0.018)。结论 早期肠内营养联合益生菌能够显著增强重型颅脑损伤患者的营养和免疫功能,同时可有效降低腹泻、腹胀等并发症的发生率,对提高重型颅脑损伤患者的治疗效果具有积极作用。

          Objective To explore the treatment effect of early enteral nutrition combined with probiotics in patients with severe craniocerebral injury. Methods Sixty-five patients with severe craniocerebral injury admitted into Kaifeng Central Hospital between January 2018 and January 2020 were selected as subjects to be divided into a study group (33 cases) and a control group (32 cases) according to the random number table. In addition to the conventional treatments, patients in the study group were given early enteral nutrition and probiotics, while patients in the control group were administered early enteral nutrition only. The nutritional status, immune function and complications in the two groups were compared. Results on day 14 of treatment, the levels of hemoglobin (Hb), serum total protein (TP) and albumin (ALB) in the patients of the two groups were all increased markedly, and the levels in the study group were significantly higher than that in the control group (t=4.787, 3.964 and 3.589, all P<0.001), and moreover, the levels of serum immune globulin (Ig) A, IgG and IgM in the patients of the two groups were all elevated obviously, and the levels in the study group were significantly higher than that in the control group (t=2.784, 3.325 and 2.676, P=0.007, 0.002 and 0.009) . The occurrence rate of complications in the study group was 6.06%, which was significantly lower than 28.13% in the control group (χ2=5.626, P=0.018). Conclusion In the treatment of severe craniocerebral injury, early enteral nutrition combined with probiotics can significantly enhance the nutrition and immune functions of such patients, and effectively reduce the occurrence of complications such as diarrhea, bloating and etc., playing a positive role in improving the therapeutic effect for such patients.
