目的 探究尺骨茎突与乙状切迹骨折对桡骨远端骨折患者腕关节功能的影响。方法 选取2018年1月至2019年7月郑州市骨科医院收治的70例桡骨远端骨折患者作为研究对象,于切开复位内固定术后随访12个月,参照Gartland-Werley评分标准评估腕关节功能恢复情况,并根据腕关节功能恢复程度进行分组,对比腕关节功能恢复不同程度患者尺骨茎突与乙状切迹骨折及愈合情况,分析尺骨茎突与乙状切迹骨折对桡骨远端骨折患者腕关节功能恢复的影响。结果 腕关节功能恢复不同程度患者单因素分析结果显示,尺骨茎突骨折类型、是否行内固定治疗以及是否合并乙状切迹骨折与腕关节的功能恢复显著相关(χ2=8.103、9.041、11.684,P=0.044、0.029、0.009);多因素Logistic回归分析结果显示,尺骨茎突基底部骨折(OR=7.501,95% CI:2.755~20.421)、未行内固定治疗(OR=2.735,95% CI:1.244~6.013)以及合并乙状切迹骨折(OR=8.109,95% CI:1.352~48.640)是影响腕关节功能恢复的独立危险因素。结论 尺骨茎突基底部骨折、未行内固定治疗以及合并乙状切迹骨折可影响桡骨远端骨折患者切开复位内固定术后腕关节的功能恢复,对于合并尺骨茎突与乙状切迹骨折者应予以重视并采取积极有效的治疗方法进行干预。 |

【Abstract】Objective To explore the influence of ulnar styloid fracture and sigmoid notch fracture on the wrist function of patients with distal radius fracture. Methods Seventy patients with distal radius fracture admitted into Zhengzhou Orthopedics Department between January 2018 and July 2019 were selected as subjects. The follow-up was conducted 12 months after the open reduction and internal fixation surgery, and the wrist function was evaluated according to the Gartland-Werley score. Based on the recovery of wrist function, the patients were divided into groups to compare the fracture and healing condition of ulnar styloid process and sigmoid notch in patients with different recovery degrees of wrist functions, and analyze the influence of the fracture of ulnar styloid process and sigmoid notch on the wrist function recovery of patients with distal radius fracture. Results The results of single-factor analysis of patients with different recovery degrees of wrist functions showed a significant correlation between the functional recovery of wrist joint and the fracture type of ulnar styloid process, whether receiving internal fixation therapy or not, and whether there was sigmoid notch fracture or not (χ2=8.103, 9.041 and 11.684, P=0.044, 0.029 and 0.009). The Logistic regression analysis of multiple factors showed that basilar fracture of ulnar styloid process (OR=7.501, 95% CI: 2.755-20.421), no internal fixation therapy (OR=2.735, 95% CI: 1.244-6.013) and occurrence of sigmoid notch fracture (OR=8.109, 95% CI: 1.352-48.640) were the independent risk factors affecting the functional recovery of wrist joint. Conclusion Basilar fracture of ulnar styloid process, without receiving the internal fixation therapy and occurrence of sigmoid notch fracture may affect the functional recovery of wrist joint of patients with distal radius fracture after the open reduction and internal fixation surgery. For such patients complicated with fracture of ulnar styloid process and sigmoid notch, special attention and effective interventions should be paid and taken in a timely fashion. |
