【摘要】 目的 分析异型聚氨酯泡沫敷料在大面积烧伤患者头部供皮区创面的应用效果。方法 选取2016年1月至2019年1月沈阳七三九医院收治的110 例大面积烧伤并行头部皮瓣移植患者作为研究对象,按照治疗方法将其分为观察组(55 例)与对照组(55 例), 观察组患者头部供皮区创面采用异型聚氨酯泡沫敷料覆盖包扎,对照组患者头部供皮区创面采用无菌纱布覆盖包扎, 对比观察两组患者头部供皮区创面愈合时间?相邻两次取皮手术间隔时间、创面疼痛程度、感染情况以及创面色素沉着与瘢痕增生情况?结果 观察组患者头部供皮区创面愈合时间、相邻两次手术间隔时间均明显短于对照组(t=16.304、10.964, P均<0.001); 术后第2?4?7天,观察组患者头部供皮区创面视觉模拟评分法(VAS)评分均明显低于对照组(t=35.481、30.419、59.724,P均<0.001);治疗期间,两组患者头部供皮区创面分泌物细菌培养均为阴性, 未出现感染;1年后随访,观察组患者轻度色素沉着率?轻度瘢痕增生率均明显低于对照组(χ2 =7.122、7.316, P=0.008、0.007)。结论 异型聚氨酯泡沫敷料可促进大面积烧伤患者头部供皮区创面愈合,减轻创面疼痛,减少色素沉着与瘢痕增生,疗效显著。 |

【Abstract】 Objective To analyze the effect of shaped polyurethane foam dressing on healing the donor site wound on scalp. Methods 110 patients with extensive burns who underwent flap transplantation with scalp flaps in Shenyang 739 Hospital between January 2016 and January 2019 were enrolled, and divided into observation group (55 cases) and control group (55 cases) based on the treatments they received. The donor site wounds on scalp of patients in observation group were covered with shaped polyurethane foam dressing, while the donor site wounds on scalp of patients in control group were covered with sterile gauze. Wound healing time of donor site, interval between two adjacent skin removal operations, woundpain degree, infection status, wound hyperpigmentation and scar hyperplasia, were compared between the two groups.Results The healing time of scalp donor site wounds and the interval between two adjacent skin removal operations in observation group were obviously shorter than control group (t = 16.304, 10.964, both P < 0.001). On day 2, 4 and 7 after surgery, the VAS scores on the scalp donor sites in observation group were all significantly lower than control group(t =35.481, 30.419 and 59.724, all P <0.001). During the course of treatment, the wound secretions from scalp donor sites were all tested negative in the two groups, and no infection occurred. At one-year follow-uthe rate of mild hyperpigmentation and the rate of mild scar hyperplasia in observation group were markedly lower than control group (χ2 =7.122 and 7.316, P =0.008 and 0.007). Conclusion Shaped polyurethane foam dressing can promote wound healing of scalp donor sites of patients with extensive burns, alleviate wound pain, and reduce hyperpigmentation and scar hyperplasia. |
