| 【摘要】 目的 研究手法复位配合新型小夹板配合新型小夹板治疗肱骨近端骨折的临床效果。方法 选取2019年1月至2020年1月商水县人民医院收治的100例肱骨近端骨折患者作为研究对象，并按照治疗方法将其分为新型小夹板组与传统小夹板组，每组50例。新型小夹板组在手法复位的基础上给予新型小夹板治疗，传统小夹板组在手法复位的基础上给予手法复位配合传统小夹板治疗，对比观察手法复位配合新型小夹板两组患者的肩关节功能、骨折愈合时间、临床疗效及患者满意度。结果 治疗后6周，新型小夹板组患者肩关节疼痛、肌力、局部形态、活动范围及日常生活活动能力评分均明显高于传统小夹板组（t=3.326、4.018、3.064、3.914、3.126，P=0.001、P＜0.001、P=0.003、P＜0.001、P=0.002），骨折愈合时间明显短于传统小夹板组（t=9.139，P＜0.001），临床疗效及患者满意度明显优于传统小夹板组（Z=-2.022、2.908，P=0.043、0.004）。结论 与手法复位配合传统小夹板相比，手法复位配合新型小夹板治疗肱骨近端骨折的愈合时间更短，肩关节功能恢复更好，疗效显著，值得临床推广使用。 |

| 【Abstract】 Objective To study the clinical efficacy of manual reduction combined with novel small splint in the treatment of proximal humeral fracture.Methods 100 patients with proximal humeral fractures, admitted to Shangshui People’s Hospital from January 2019 to January 2020, were enrolled and divided into novel splint group (n = 50) and traditional splint group (n = 50) according to the treatment they received.In addition to manual reduction, patients in novel splint group were treated with novel small splints, whereas patients in traditional splint group were treated with traditional small splints.The shoulder joint function, fracture healing time, clinical efficacy and patients’ satisfaction were compared between the two groups.Results After 6 weeks of treatment, the scores of shoulder joint pain, muscle strength, local morphology, range of motion, and activities of daily living in novel splint group were all significantly higher than traditional splint group（t=3.326、4.018、3.064、3.914、3.126，P=0.001、P＜0.001、P=0.003、P＜0.001、P=0.002）fracture healing time in novel splint group was significantly shorter than traditional splint group （t=9.139，P＜0.001）and the clinical efficacy and patients’ satisfaction in novel splint group were also significantly superior to traditional splint group（Z=-2.022、2.908，P=0.043、0.004）.Conclusion Compared with manual reduction combined withtraditional small splint, manual reduction plus novel small splint in the treatment of proximal humeral fracture can realize shorter fracture healing time, better recovery of shoulder joint function and remarkable clinical efficacy, deserving to be promoted in clinical practice. |
