【摘要】 目的 分析探讨胫骨远端骨折切开复位内固定术后切口感染的相关危险因素。方法 选取2018 年1 月至2020 年4 月驻马店市第一人民医院收治的265 例行切开复位内固定术治疗的胫骨远端骨折患者作为研究对象,根据是否发生术后切口感染将其分为感染组与非感染组,分析切口感染的相关危险因素。结果 265 例患者均顺利完成手术,其中术后发生切口感染者23 例,设为感染组; 未发生切口感染者242 例,设为非感染组。多因素Logistic 回归分析结果显示,高龄、合并有糖尿病、开放性骨折、致伤暴力程度高、伤口污染是胫骨远端骨折切开复位内固定术后切口感染的独立危险因素(95% CI: 1.072~1.244、2.450~25.684、0.998~9.026、1.769~20.290、0.003~0.047,P = 0.000、0.001、0.048、0.004、0.000)。结论 高龄、合并有糖尿病、开放性骨折、致伤暴力程度高、伤口污染是胫骨远端骨折切开复位内固定术后切口感染的独立危险因素,术前应及早对危险因素进行干预,以预防切口感染的发生。 |

【Abstract】 Objective To analyze the risk factors for incision infection after open reduction and internal fixation for distal tibial fractures. Methods 265 patients with distal tibial fractures, treated with open reduction and internal fixation in The First People’s Hospital of Zhumadian from January 2018 to April 2020, were enrolled and divided into infection group and non?infection group based on whether there was incision infection after surgery, to analyze related risk factors for incision infection. Results All the 265 patients underwent surgery successfully. Among them, 23 cases developed incision infection after surgery were set as infection group, and the other 242 cases without incision infection were set as non-infection group. The results of multivariate Logistic regression analysis showed that advanced age, complicated with diabetes, open fractures, high degree of traumatic force, and wound contamination were independent risk factors for incision infection after open reduction and internal fixation for distal tibial fractures(95% CI: 1.072~1.244、2.450~25.684、0.998~9.026、1.769~20.290、0.003~0.047,P = 0.000、0.001、0.048、0.004、0.000). Conclusion Advanced age, complicatedwith diabetes, open fractures, high degree of traumatic force, and wound contamination are independent risk factors for inci?sion infection after open reduction and internal fixation for distal tibial fractures. These risk factors should be intervenedbefore surgery as soon as possible to prevent incision infection. |
