| 【摘要】 目的 探讨超脉冲CO2点阵激光联合胶原贴敷料治疗面部痤疮瘢痕的临床效果。方法 选取2020年1月至2021年1月宜阳县人民医院收治的60例面部痤疮瘢痕患者作为研究对象，并按照随机数表法将其随机分为观察组和对照组，每组30例，观察组患者采用超脉冲CO2点阵激光联合胶原贴敷料治疗，对照组患者单纯采用超脉冲CO2点阵激光治疗，对比两组患者面部美观度与皮肤屏障功能变化情况、临床疗效以及不良反应发生情况。结果 治疗结束后，观察组患者面部美观度毛孔值、纹理值以及皮肤屏障功能经皮水分丢失（TEWL）、pH值、皮肤红斑指数明显低于对照组（t=13.490、8.883、2.204、2.692、5.931，P＜0.001、P＜0.001、P=0.032、P=0.009、P＜0.001），角质层含水量明显高于对照组（t=2.752，P=0.008）；观察组患者中痊愈10例、显效12例、有效6例、无效2例，明显优于对照组患者的痊愈6例、显效7例、有效9例、无效8例（Z=-2.285，P=0.022）；治疗期间，观察组患者不良反应发生率为6.67%，明显低于对照组患者的不良反应发生率30.00%（χ2=5.455，P=0.020）。结论 超脉冲CO2点阵激光联合胶原贴敷料治疗痤疮瘢痕，可明显改善面部美观度和皮肤屏障功能，疗效显著，且胶原贴敷料可明显降低CO2点阵激光治疗造成的不良反应发生率。 |

| 【Abstract】 Objective To study the clinical efficacy of ultrapulse fractional CO2 laser combined with collagen dressing in the treatment of acne scars on the face. Methods 60 patients with facial acne scars admitted to Yiyang County People’s Hospital between January 2020 and January 2021, were enrolled and divided, using random number table, into study group (n =30) and control group (n =30). Patients in study group were treated with ultrapulse fractional CO2 laser combined with collagen dressing, while patients in control group were simply treated with ultrapulse fractional CO2 laser.
The changes of facial aesthetics and skin barrier functions, clinical efficacy and occurrence of adverse reactions were observed and compared between the two groups. Results After treatment, the values of facial skin pore, texture, transepi-dermal water loss (TEWL) and pH, and erythema index of patients in study group were all obviously lower than control group (t =13.490, 8.883, 2.204, 2. 692 and 5.931, P < 0.001, P < 0.001, P = 0.032, P = 0.009 and P < 0.001),and the water content of stratum corneum in study group was significantly higher than control group (t = 2.752, P =0.008). In study group, 10 cases were cured, 12 cases were markedly effective, 6 cases were effective and 2 case was ineffective, and the efficacy was significantly better than control group (Z = -2.285, P =0.022) in which 6 cases were cured,7 cases were markedly effective, 9 cases were effective and 8 cases were ineffective. During the course of treatment, the incidence of adverse reactions in study group was 6.67%, which was markedly lower than the corresponding 30.00% in control group (χ2 = 5.455, P =0.020). Conclusion The combined application of ultrapulse fractional CO2 laser and collagen dressing in treating acne scar on the face can significantly improve facial aesthetics and skin barrier functions, and realize remarkable therapeutic benefits. Moreover, collagen dressing can greatly reduce the incidence of adverse reactions caused by fractional CO2 laser. |
