          【摘要】 目的 探究重组人生长激素治疗重度烧伤的临床效果及其对生长激素(GH)、前白蛋白(PAB)、血清总蛋白(TP)水平的影响。方法 选取2019年2月至2020年2月中国人民解放军联勤保障部队第九九〇医院收治的56例重度烧伤患者作为研究对象,并按照随机数表法将其随机分为观察组(28例)与对照组(28例),观察组患者在常规综合治疗的基础上联合应用重组人生长激素治疗,对照组患者单纯采用常规综合治疗,对比观察两组患者GH、PAB、TP水平变化情况以及创面愈合时间、住院时间。结果 治疗10 d后,观察组患者GH 水平为(15.34 ±3.19) μg/L、PAB 水平为(3.31±0.32) g/L、TP水平为(57.23 ±6.85) g/L,明显高于对照组患者的GH水平(6.15±1.62)μg/L、PAB水平(2.14±0.23) g/L、TP水平(46.43±5.62) g/L (t =13.592、15.710、6.450, P均<0.001);观察组患者供皮区创面愈合时间为(8.13±2.05)d、植皮区创面愈合时间为(25.24 ±5.53) d、住院时间为(27.53±5.63) d,明显短于对照组患者的供皮区创面愈合时间(14.23±2.12)d、植皮区创面愈合时间(34.31±6.41) d、住院时间(38.21±6.71) d (t = 10. 945、5.669、6.452,P均<0.001)。结论 重组人生长激素可有效提高重度烧伤患者体内GH、PAB、TP的表达水平,促进创面愈合,缩短住院时间,疗效显著,值得临床推广应用。

          【Abstract】 Objective To study the clinical efficacy of recombinant human growth hormone in treating severe burns and its influence on levels of growth hormone (GH), prealbumin (PAB) and total protein (TP). Methods 56 patients with severe burns, admitted to The 990th Hospital of PLA Joint Logistics Support Force from February 2019 to February 2020, were enrolled and divided into study group (n=28) and control group (n=28) using the random number table. Patients in study group were treated with recombinant human growth hormone in addition to conventional comprehensive treatments,whereas patients in control group only received conventional comprehensive treatments. Changes of GH, PAB and TP,wound healing time and length of stay were compared between the two groups. Results After 10 days of treatment,the levels of GH, PAB and TP were respectively (15.34±3.19)μg/L,(3.31±0.32)g/Land (57.23±6.85)g/L in study group,which were significantly higher than the corresponding (6.15±1.62) μg/L,(2.14 ±0.23)g/L and (46.43 ±5.62) g/L in control group (t=13.592,15.710 and 6.450,all P<0.001). The wound healing time ofdonor sites, wound healing time of implantation sites and length of stay were respectively (8.13±2.05) d,(25.24±5.53) d and (27.53±5.63) d in study group,which were remarkably shorter than the corresponding (14.23±2.12)d,(34.31±6.41) d and (38. 21±6.71) d in control group (t =10.945, 5.669 and 6.452, all P<0.001). Conclusion Recombinant human growth hormone can effectively improve the levels of GH, PAB and TP in patients with severe burns,promote wound healing and shorten length of stay, and the clinical efficacy was definite,deserving to be promoted in clinical practice.
