【摘要】 目的 系统分析细胞毒性 T 淋巴细胞相关抗原4免疫球蛋白 (CTLA4Ig)基因转染猪皮敷料对烧伤创面的治疗效果。方法 计算机检索中文科技期刊数据库 (维普)、中国期刊全文数据库(中国知网)、中国学术期刊数据库 (万方)、PubMed、The Cochrane Library等数据库建库至2020年12月公开发表的CTLA4Ig 基因转染猪皮敷料治疗烧伤创面的相关文献,并根据纳入与排除标准筛选符合标准的文献进行质量评价及Meta分析。结果 共纳入符合标准的文献5篇,包含烧伤患者368例。Meta分析结果显示,创面行CTLA4Ig 基因转染猪皮敷料治疗的烧伤患者创面愈合时间明显短于其他疗法(MD=-4.01,95% CI 为 -5.08~-2.95,P < 0.001)色素沉着/脱失发生率明显低于其他疗法 (RR=0.16,95%CI为0.08~0.33,P < 0.001),而创面感染率和瘢痕增生 情况与其他疗法无明显差异 (RR = 0.14、0.61,95%CI为 0.02~1.10、0.37~1.01,P = 0.060、0.050)。结论 与其他疗法相比, CTLA4Ig 基因转染猪皮敷料更能有效缩短烧伤创面愈合时间、减少色素沉着/脱失。 |

【Abstract】 Objective To systematically analyze the clinical efficacy of cytotoxic T lymphocyte associated antigen-4 immunoglobulin ( CTLA4Ig) gene transfected pigskin in the treatment of burns. Methods Computer-based literature research was conducted in such databases as China Science and Technology Journal Database (CQVIP), China National Knowledge Infrastructure (CNKI), China Science Periodical Database (Wanfang), PubMed and The Cochrane Library to retrieve papers studying CTLA4Ig in the treatment of burn wounds published between the inception of the databases and December 2020, and quality assessment and Meta-analysis were carried out on the qualified papers included based on the inclusion and exclusion criteria. Results A total of 5 papers screened out from the retrieved literature were included intothe study, with 368 burn patients involved. Meta-analysis showed that, compared with other therapies, the wound healing time of burn patients treated with CTLA4Ig gene transfected pigskin was obviously shorter (MD =-4.01,95% CI 为 -5.08~-2.95,P < 0.001),incidences of hyperpigmentation / depigmentation were markedly lower(RR=0.16,95%CI 为 0.08~0.33,P < 0.001); while there was no significant difference in terms of wound infection incidence and occurrence of hyper- plasia scar(RR = 0.14、0.61,95%CI为 0.02~1.10、0.37~1.01,P = 0.060、0.050). Conclusion Compared with other therapies in the treatment of burns, CTLA4Ig gene transfected pigskin can significantly shorten the wound healing time, and reduce hyperpigmentation and depigmentation. |
