          【摘要】 目的 探讨股前外侧游离皮瓣在上肢高压电击伤创面修复中的临床效果? 方法 给予 2018 年 5 月至 2021 年 10 月郑州市第一人民医院收治的 27 例上肢高压电击伤患者行股前外侧游离皮瓣修复创面, 观察患者手术时间、术中出血量、住院时间、术后皮瓣成活情况、并发症发生情况以及患肢功能恢复情况? 结果 27 例患者均顺利完成股前外侧游离皮瓣移植手术, 手术时间 260~735 min, 术中出血量 240~650 ml, 住院时间 35~160 d, 除 3 例患者因皮瓣远端部分坏死行自体皮移植后创面愈合外, 其余 24 例患者术后皮瓣均完全成活(88. 89%), 愈后皮肤外观良好, 且所有患者均未出现血栓栓塞、创面感染等并发症? 术后随访 6 个月, 患肢功能恢复为优者 7 例、良者 12 例、可者 5 例、差者 3 例, 优良率为 70. 37%? 结论 股前外侧游离皮瓣修复上肢高压电击伤创面, 皮瓣成活率较高, 有利于上肢功能恢复, 且并发症发生率较低, 安全性较好?

          【Abstract】 Objective To study the therapeutic effect of free anterolateral thigh flap transplantation on upper limb wounds caused by high voltage electricity. Methods Free anterolateral thigh flap transplantation was performed on 27 patients with high voltage electric injuries on their upper limbs, admitted to First People’s Hospital of Zhengzhou City from May 2018 to October 2021, to repair their wounds, and the operation time, intraoperative blood loss, length of stay, flap survival after operation, occurrence of complications and functional recovery of affected limbs were observed. Results All the 27 patients went through the free anterolateral thigh flap transplantation successfully. The operation time was 260-735 min, the intraoperative blood loss was 240 - 650 ml, and the length of stay was 35 - 160 days. Of the 27 patients, 3 patients received autologous skin transplantation due to partial distal flap necrosis for wound healing, and the other 24 patients (88.89%) got wound healing with the well survival of transplanted flaps, and the healed skin showed good appearance. No complications such as thromboembolism and wound infection were observed in all patients. During the six-month follow-up,the functional recovery of the affected limbs of patients was evaluated as excellent in 7 cases, good in 12 cases, fair in 5 cases, poor in 3 cases, and the excellent and good rate was 70.37%. Conclusion The free anterolateral thigh flap trans-plantation in the treatment of high voltage electrical injury on upper limbs boasts high survival rate of flaps, favoring therecovery of upper limb functions, low incidence of complications and high safety.
