          【摘要】 目的 研究分析重组牛碱性成纤维细胞生长因子凝胶联合微波照射治疗慢性难愈合烧伤创面的临床疗效。方法 选取 2019 年 3 月至 2021 年 3 月大连市友谊医院收治的 150 例慢性难愈合烧伤创面患者作为研究对象, 并按照随机数表法将其随机分为研究组 (75 例) 与对照组 (75 例), 研究组患者局部创面采用重组牛碱性成纤维细胞生长因子凝胶联合微波照射治疗, 对照组患者局部创面单纯采用重组牛碱性成纤维细胞生长因子凝胶治疗, 对比观察两组患者创面组织内血管内皮生长因子 (VEGF)、表皮生长因子 (EGF) 表达水平变化情况以及创面愈合时间。结果 治疗第 3、7、14 天, 研究组患者创面组织内 VEGF? EGF 表达水平均明显高于对照组(VEGF: t = 3.544、2.664、3.556, P < 0.001、P = 0.009、P < 0.001; EGF: t = 2.138、3.157、2.330, P =0.034、0.002、0.021); 研究组患者创面愈合时间为 (45.4 ± 10.9) d, 明显短于对照组患者的创面愈合时间(54.8 ± 12.6) d (t = 4.886, P < 0.001)。结论 重组牛碱性成纤维细胞生长因子凝胶联合微波照射治疗慢性难愈合烧伤创面, 有利于提高创面组织内 VEGF、EGF 的表达水平, 缩短创面愈合时间, 疗效显著。

          【Abstract】 Objective To study the clinical efficacy of recombinant bovine basic fibroblast growth factor (rb-bFGF) gel combined with microwave irradiation in the treatment of chronic refractory burn wounds. Methods 150 patients with chronic refractory burn wounds, admitted in Dalian Friendship Hospital from March 2019 to March 2021, were enrolled as study subjects, and were divided, using the random number table, into study group (n = 75) and control group (n = 75). Patients in the study group were treated with rb-bFGF gel combined with microwave irradiation, while patients in the control group were treated with rb-bFGF gel alone. The changes of expression levels of vascular endothelial growth factor (VEGF) and epidermal growth factor (EGF) in wound tissues, and the wound healing time were compared between the two groups. Result On day 3, 7 and 14 of treatment, the expression levels of VEGF and EGF in the wound tissues of patients were all significantly higher in the study group compared with the control group (VEGF: t = 3.544, 2.664 and 3.556, P < 0.001,P = 0.009, P < 0.001; EGF: t = 2.138, 3?? 157 and 2.330, P = 0.034, 0.002 and 0.021). The wound healing time was (45.4 ± 10. 9) days in the study group, significantly shorter than the corresponding (54.8 ± 12.6) days in the control group (t = 4.886, P < 0.001). Conclusion The rb-bFGF gel combined with microwave irradiation in the treatment of chronic refractory burn wounds can increase the expression levels of VEGF and EGF in the wound tissues and shorten wound healing time, and the therapeutic effect is definite.
