          【摘要】 目的 研究探讨中药抗休克合剂对Ⅲ度烫伤大鼠早期肺损伤的影响机制。方法 选取32只健康SD大鼠适应性饲养1周后,按照随机数表法将其随机分为假伤组、烫伤组、LR 组、LR +中药组,每组8只,其中假伤组大鼠备皮后做假伤处理,烫伤组大鼠备皮后建立Ⅲ度烫伤模型,LR 组大鼠备皮后建立Ⅲ度烫伤模型并予以乳酸钠林格注射液 (LR) 补液,LR +中药组大鼠备皮后建立Ⅲ度烫伤模型并予以LR补液联合中药抗休克合剂灌胃,对比观察各组大鼠血清炎症因子水平以及肺组织湿重/干重 (W/ D) 比值、氧化应激水平与 p38 丝裂原活化蛋白激酶 (p38MAPK)、胞外信号调节激酶 1 / 2 (ERK1 / 2) 通路活化程度。结果 烫伤后各组大鼠血清肿瘤坏死因子-α (TNF-α)、白细胞介素 (IL) -10、IL-18、髓过氧化物酶 (MPO)、丙二醛 (MDA) 水平以及 W/ D比值均明显升高 (F = 112.352、80.455、40.716、79.321、270.356、60.784,P 均 < 0.001),超氧化物歧化酶(SOD) 水平明显降低 (F = 157.977,P < 0.001),p38MAPK 与 ERK1 / 2 通路活化程度均明显增加 (F = 402.669、3764.000,P 均 < 0.001),且 LR + 中药组大鼠血清 TNF-α、IL-18、MPO、MDA 水平以及 p38MAPK 与 ERK1 / 2 通路活化程度均明显低于烫伤组和 LR 组 (LR + 中药组比烫伤组: q = 11.070、9.439、16.000、23.970、30.480、100.500,P 均 < 0.001; LR + 中药组比 LR 组: q = 4.579、5.864、6.286、9.444、10.980、51.310,P = 0.015、P = 0.002、P = 0.001、P < 0.001、P < 0.001、P < 0.001),IL-10、SOD 水平均明显高于烫伤组和 LR 组 (LR + 中药组比烫伤组: q = 10.490、16.240,P 均 < 0.001; LR + 中药组比 LR 组: q = 6.583、6.339,P < 0.001、P =0.001)。结论 中药抗休克合剂可通过降低机体炎症反应及氧化应激损伤程度、抑制 p38MAPK 和 ERK1 / 2 信号通路磷酸化而减轻Ⅲ度烫伤大鼠早期肺组织损伤。

          【Abstract】 Objective To study the mechanism of action of Chinese medicine anti-shock mixture on early lung injury of rats with Ⅲ-degree scalds. Methods 32 healthy SD rats, after adaptive feeding for one week, were divided,using the random number table, into sham group (n = 8), scald group ( n = 8), LR group ( n = 8) and LR + Chinese medicine group (n = 8). After skin preparation, rats in sham group weren’t made injury, rats in scald group were made Ⅲ-degree scald model, rats in LR group were made Ⅲ-degree scald model and given fluid replacement with sodium lactate Ringer’s injection (LR), while rats in LR + Chinese medicine group were also made Ⅲ-degree scald model and given fluid replacement with sodium lactate Ringer’s injection (LR) plus intragastric injection of Chinese medicine anti-shock mixture. The levels of serum inflammatory factors, lung wet weight / dry weight (W/ D) ratio, oxidative stress level, and the activation of p38 mitogen-activated protein kinase (p38MAPK) and extracellular signal-regulated kinase 1 / 2 (ERK1 / 2) pathways were compared among the four groups. Results After being scalded, the levels of serum tumor necrosis factor -α (TNF-α), interleukin-10 (IL-10), interleukin-18 (IL-18), myeloperoxidase (MPO), malondialdehyde (MDA), and the W/ D ratio of rats were all significantly elevated in the four groups (F = 112.352, 80.455, 40.716, 79.321, 270.356 and 60.784, all P < 0.001), while the level of superoxide dismutase ( SOD) were obviously decreased (F = 157.977, P <0.001), and the activation degree of p38MAPK and ERK1 / 2 pathways were markedly increased ( F = 402.669 and 3764.000, both P < 0.001). Moreover, the levels of serum TNF-α, IL-18, MPO and MDA, and the activation degree of p38MAPK and ERK1 / 2 pathways were all significantly lower in the LR + Chinese medicine group compared with scald group and LR group (LR + Chinese medicine group vs. scald group: q = 11.070, 9.439, 16.000, 23.970, 30.480 and 100.500, all P < 0.001; LR + Chinese medicine group vs.LR group: q = 4.579, 5.864, 6.286, 9.444, 10.980 and 51.310, P = 0.015, P = 0.002, P = 0.001, P < 0.001, P < 0.001, P < 0.001 ), and the levels of IL-10 and SOD were obviously higher in the LR + Chinese medicine group compared with scald group and LR group (LR + Chinese medicine group vs. scald group: q = 10.490 and 16.240, both P < 0.001; LR + Chinese medicine group vs.LR group: q = 6.583 and 6.339, P < 0.001, P = 0.001). Conclusion Chinese medicine anti-shock mixture can alleviate the early lung injury of rats with Ⅲ-degree scalds by decreasing the inflammatory responses and oxidative stress damage, inhibiting the phosphorylation of p38MAPK and ERK1 / 2 signal pathways.
