          【摘要】 目的 研究分析复方黄柏液涂剂联合胫骨横向骨搬移在 Wagner 4 级糖尿病足中的应用效果。方法 选取 2019 年 12 月至 2021 年 12 月信阳职业技术学院附属医院收治的 70 例 Wagner 4 级糖尿病足患者作为研究对象, 按照随机数表法将其随机分为观察组 (35 例) 和对照组 (35 例), 观察组患者行复方黄柏液涂剂联合胫骨横向骨搬移治疗, 对照组患者行康复新液联合胫骨横向骨搬移治疗, 对比观察两组患者血清生长因子及炎症因子水平、临床疗效、创面愈合时间与不良反应发生情况。结果 术后 1 个月, 观察组患者血清血管内皮生长因子 (VEGF)、碱性成纤维细胞生长因子 ( bFGF)、表皮生长因子 (EGF) 水平均明显高于对照组 ( t = 2.029?18.039、23.895, P = 0.023、P < 0.001、P < 0.001), C 反应蛋白 (CRP)、白细胞介素-6 ( IL-6)、肿瘤坏死因子-α (TNF-α) 水平均明显低于对照组 ( t = 9.260、9.224、8.849, P 均 < 0.001); 术后 3 个月, 观察组患者治疗总有效率明显高于对照组 ( χ2= 5.285, P = 0.022); 观察组患者创面愈合时间明显短于对照组 ( t = 2.212,P = 0.030); 术后随访 6 个月, 观察组患者不良反应发生率与对照组无明显差异 ( χ2= 0.565, P = 0.452)。结论 复方黄柏液涂剂联合胫骨横向骨搬移治疗 Wagner 4 级糖尿病足, 能够明显提高 VEGF、bFGF、EGF 等生长因子表达水平, 降低机体炎症反应程度, 促进创面愈合, 疗效显著。

          【Abstract】 Objective To analyze the clinical efficacy of Fufang Huangbaiye Tuji combined with tibial transverse transport in the treatment of diabetic foot of Wagner grade 4. Methods 70 patients with diabetic foot of Wagner grade 4, admitted into The Affiliated Hospital of Xinyang Vocational and Technical College between December 2019 and December 2021, were enrolled as research subjects, and divided into study group ( n = 35) and control group ( n = 35) using the random number table. Patients in the study group were treated with Fufang Huangbaiye Tuji combined with tibial transverse transport, while patients in the control group were treated with Kangfuxin Solution (Potion of Recovery) combined with tibial transverse transport. The levels of serum growth factors and inflammatory factors, clinical efficacy, wound healing time and occurrence of adverse reactions of patients were compared between the two groups. Results One month after operation, the levels of serum vascular endothelial growth factor (VEGF), basic fibroblast growth factor ( bFGF) and epidermal growth factor (EGF) of patients were all obviously higher in the study group compared with the control group (t = 2.029, 18.039 and 23.895, P = 0.023, P < 0.001, P < 0.001), and the levels of C-reactive protein (CRP), interleukin-6 ( IL-6) and tumor necrosis factor-α (TNF-α) of patients were all markedly lower in the study group (t = 9.260, 9.224 and 8.849, all P < 0.001). Three months after operation, the total effective rate was significantly higher in the study group compared with the control group (χ2= 5.285, P = 0.022). The wound healing time of patients was obviously shorter in the study group compared with the control group (t = 2.212, P = 0.030). During the follow-up six months after operation, the incidence of adverse reactions of patients in the study group showed no statistically significant difference compared with that in the control group (χ2= 0.565, P = 0.452). Conclusion The combined application of Fufang Huangbaiye Tuji and tibial transverse transport in the treatment of diabetic foot of Wagner grade 4 can significantly increase the expression levels of VEGF, bEGF and EGF, decrease inflammatory responses, and promote wound healing, and the clinical efficacy is definite.
