【摘要】 目的 探讨改良包扎法在臀部及会阴部Ⅱ度烧伤创面治疗中的应用效果。 方法 选取 2019 年 4 月至 2020 年 4 月南京鼓楼医院集团宿迁医院收治的 39 例烧伤患者 (均包含臀部及会阴部Ⅱ度烧伤) 作为研究对象, 按照不同包扎方法将其分为改良包扎组 (20 例) 和传统包扎组 (19 例)。 改良包扎组患者臀部及会阴部创面均匀涂抹湿润烧伤膏后依次覆盖银离子抗菌敷料、无菌干纱布及自制可拆卸分体式开裆裤样敷料固定件包扎固定; 传统包扎组患者臀部及会阴部创面均匀涂抹湿润烧伤膏后覆盖银离子抗菌敷料包扎固定, 对比观察两组患者臀部及会阴部创面感染情况、换药次数、愈合时间以及愈后皮肤瘢痕评分及治疗费用。 结果 改良包扎组患者臀部及会阴部创面感染例数及换药次数均明显少于传统包扎组 (t =7.653、8.416, P =0.006、P <0.001), 愈合时间明显短于传统包扎组 (t =3.795, P <0.001), 温哥华瘢痕量表 (VSS) 评分明显低于传统包扎组 (t =4.269,P <0.001), 治疗费用与传统包扎组无明显差异 (Z = - 0.927, P = 0.365)。 结论 臀部及会阴部Ⅱ度烧伤创面采用自制可拆卸分体式开裆裤样敷料固定件进行包扎固定, 可有效减少创面感染风险及换药次数, 促进创面愈合, 减轻愈后皮肤瘢痕增生, 疗效显著, 值得临床推广使用。 |

【Abstract】 Objective To study the application effect of modified bandaging in second-degree burns at buttocks and perineum. Methods 39 burn patients (all with second-degree burns at buttocks and perineum) admitted into Nanjing Drum Tower Hospital Group Suqian Hospital between April 2019 and April 2020 were enrolled as research subjects, and divided into modified bandaging group (n = 20) and routine bandaging group (n = 19) based on different bandaging methods they received. The burn wounds of patients at buttocks and perineum were treated with the external application of MEBO followed by the successive covering and bandaging of silver ion antibacterial dressing, sterile dry gauze and self-made detachable split-pants-like dressing fastener in the modified bandaging group, while the burn wounds of patients in the routine bandaging group were treated with the external application of MEBO followed by the bandaging of silver ion antibacterialdressing alone. The wound infection at buttocks and perineum, time of dressing change, wound healing time, scar scale scores of healed skin and treatment cost of patients were compared between the two groups. Results Compared with the routine bandaging group, the number of cases with wound infection at buttocks and perineum and the frequency of dressing change in the modified bandaging group were both obviously less (t = 7.653 and 8.416, P = 0.006, P < 0.001), the wound healing time was markedly shorter (t =3.795, P <0.001), and the Vancouver scar scale (VSS) score was obviously
lower (t =4.269, P < 0.001), though there was no statistically significant difference between the two groups in terms of treatment cost (Z = -0.927, P =0.365). Conclusion The application of self-made detachable split-pants-like dressing fastener in the burn wounds at buttocks and perineum can effectively decrease the infection risk and the frequency of dressing change, promote wound healing, alleviate scar hyperplasia of healed skin, and the clinical efficacy is definite, deserving to be promoted in clinical practice. |
