【摘要】 目的 分析乙醇冰袋在四肢骨折患者术前冷敷中的应用效果。 方法 选取2020 年1 月至2021 年6月信阳一五四医院收治的 200 例四肢闭合性骨折患者作为研究对象, 按照随机数表法将其随机分为观察组 (100例) 与对照组 (100 例), 观察组患者骨折部位采用乙醇冰袋冷敷处理, 对照组患者骨折部位采用生理盐水冰袋冷敷处理, 对比观察两组患者患肢肿胀与疼痛程度、血液流变学指标水平以及术前并发症发生情况。 结果 冷敷处理 3 d 后, 观察组患者患肢肿胀及疼痛程度均明显轻于对照组 (肿胀: Z = - 2.342, P = 0.019; 疼痛: t =9.061, P <0.001), 全血黏度及红细胞压积均明显低于对照组 (t =5.340、3.796, P 均 <0.001); 观察组患者术前并发症发生率为 5.0%, 与对照组患者的术前并发症发生率 10.0% 无明显差异 (χ 2 = 1.802, P = 0.179)。 结论 与生理盐水冰袋相比, 乙醇冰袋更能有效缓解四肢骨折患者术前局部肿胀及疼痛程度, 降低全血黏度与红细胞压积, 值得临床推广应用。 |

【Abstract】 Objective To analyze the effect of ethanol ice pack in the preoperative cold compress of patients with limb fracture. Methods 200 patients with closed limb fracture, admitted into Xinyang 154th Hospital between January 2020 and June 2021, were enrolled as research subjects, and divided into study group (n =100) and control group (n =100) using the random number table. The fracture sites of patients in the study group were given cold compress with ethanol ice pack, while the fracture sites of patients in the control group were given cold compress with normal saline ice pack. The swelling and pain degree of the affected limb, levels of hemorheological indexes and occurrence of preoperative complications of patients were compared between the two groups. Results Three days after cold compress, the swelling and pain degree of the affected limbs of patients were obviously milder in the study group compared with the control group (swelling: Z =-2.342, P =0.019, pain: t = 9.061, P < 0.001 ), and the whole blood viscosity and hematocrit were also markedly lower in the study group (t =5.340 and 3.796, both P <0.001). The incidence of preoperative complications of patients in the study group was 5.0%, showing no statistically significant difference compared with the corresponding 10.0% in the control group (χ 2 =1.802, P =0.179). Conclusion Compared with the normal saline ice pack, the ethanol ice pack is more effective in alleviating the preoperative local swelling and pain of patients with limb fracture, and decreasing their whole blood viscosity and hematocrit, deserving to be promoted in clinical practice. |
