          【摘要】 目的 探讨分析股骨颈骨折患者术后骨折延迟愈合的危险因素及其相关性。 方法 选取 2019 年 3月至 2022 年 1 月信阳淮河骨科医院收治的 98 例拟行切开复位内固定术治疗的股骨颈骨折患者作为研究对象, 收集并统计患者性别、年龄、居住地、骨折原因、术前血清肿瘤坏死因子-α (TNF-α) 及胰岛素样生长因子-1(IGF-1) 水平等资料, 并根据股骨颈骨折术后 4 个月骨折端是否愈合将患者分为正常愈合组和延迟愈合组, 多因素 Logistic 回归分析股骨颈骨折术后骨折延迟愈合的危险因素, 点二列相关性分析各危险因素与股骨颈骨折术后骨折延迟愈合的相关性。 结果 股骨颈骨折术后 4 个月, 98 例患者中出现骨折延迟愈合 16 例 (16.33%), 设为延迟愈合组; 骨折正常愈合 82 例 (83.67%), 设为正常愈合组。 多因素 Logistic 回归分析结果显示, 血清 TNF-α高水平、IGF-1 低水平是股骨颈骨折术后骨折延迟愈合的独立危险因素 (95% CI 为 1.146 ~ 1.774、0.936 ~0.994, P =0.001、0.018); 点二列相关性分析结果显示, 术前血清 TNF-α 水平与股骨颈骨折术后骨折延迟愈合呈显著正相关性 (r =0.702, P <0.001), 术前血清 IGF-1 水平与股骨颈骨折术后骨折延迟愈合呈显著负相关性(r = -0.569, P <0.001)。 结论 股骨颈骨折术后骨折延迟愈合与术前血清 TNF-α 及 IGF-1 水平密切相关。

          【Abstract】 Objective To analyze the risk factors of delayed fracture healing after femoral neck fracture surgery and their correlations. Methods 98 patients with femoral neck fracture, who were admitted into Xinyang Huaihe Orthopedic Hospital to undergo the incision reduction and internal fixation between March 2019 and January 2022, were enrolled as research subjects, and their data were collected including sex, age, place of residence, cause of fracture, and levels of pre-operative tumor necrosis factor-α (TNF-α) and insulin-like growth factor-1 (IGF-1). Based on whether the fracture site was healed four months after the femoral neck fracture surgery, the patients were divided into normal healing group and delayed healing group. Multivariate Logistic regression analysis was conducted to analyze the risk factors of delayed fracture healing after the femoral neck fracture surgery, and the point biserial correlation was used to measure the relationships between the risk factors and the delayed fracture healing. Results Four months after the femoral neck fracture surgery, among the 98 cases, 16 cases (16.33%) developed delayed fracture healing, being set as delayed healing group, and the other 82 cases (83.67%) realized normal fracture healing, being set as normal healing group. The results of multivariate Logistic regression analysis showed that high level of serum TNF-α and low level of IGF-1 were the independent risk factors of fracture delayed healing after the femoral neck fracture surgery (95%CI: 1.146 -1.774, 0.936 -0.994, P =0.001 and 0.018). The point biserial correlation analysis revealed that the preoperative level of serum TNF-α was positively correlated with the fracture delayed healing (r =0.702, P <0.001), and the preoperative level of serum IGF-1 was negatively associated with the fracture delayed healing (r = - 0.569, P < 0.001). Conclusion The delayed healing of fracture after the femoral neck fracture surgery was closely associated with the preoperative levels of serum TNF-α and IGF-1.
