【摘要】 目的 分析探讨不同脉冲染料激光治疗间隔时间在烧伤后增生性瘢痕患者中的应用效果。方法 选取 2019 年 2 月至 2020 年 12 月河南科技大学第一附属医院收治的 104 例烧伤后增生性瘢痕患者作为研究对象,按照不同激光治疗间隔时间将其分为 1 周组 (52 例) 和 2周组 (52 例), 1周组患者每周进行 1 次脉冲染料激光治疗, 2 周组患者每 2 周进行 1 次脉冲染料激光治疗, 对比观察两组患者瘢痕血流灌注量、临床疗效以及不良反应发生情况。结果 治疗 3 个月后, 1 周组患者瘢痕血流灌注量为 (1.49±0.37) V, 明显高于 2 周组患者的瘢痕血流灌注量 (1.32±0.23) V (t = 2.814, P = 0.006); 1 周组患者中治愈 16 例、显效 16 例、有效 7 例、无效13 例, 与 2 周组患者的治愈 20 例、显效 12 例、有效 9 例、无效 11 例无明显差异 (Z = - 0.565, P = 0.572)。治疗期间, 1 周组患者的不良反应发生率为 17.31% , 明显高于 2 周组患者的不良反应发生率 3.85% (χ2= 4.981,P = 0.026)。 结论 间隔 1 周与间隔 2 周进行脉冲染料激光治疗烧伤后增生性瘢痕的临床疗效相当, 但间隔 2 周更有利于减少瘢痕组织血流灌注, 且不良反应发生率更低。 |

【Abstract】 Objective To analyze the effects of different treatment intervals of pulsed dye laser on the treatment of hypertrophic scar after burn. Methods 104 patients with hypertrophic scar after burn, admitted to The First Affiliated Hospital of Henan University of Science and Technology from February 2019 to December 2020, were enrolled as research subjects and divided into 1-week group (n = 52) and 2-week group (n = 52) according to different laser treatment intervals they received. Patients in the 1-week group were given pulsed dye laser treatment once a week, while patients in the 2-week group were given pulsed dye laser treatment every two weeks. The scar blood perfusion volumes, clinical efficacy, and occurrence of adverse reactions were compared between the two groups. Results After three months of treatment, the scar blood perfusion volume in the 1-week group was (1.49±0.37) V, being obviously higher than the corresponding (1.32±0.23) V in the 2-week group ( t = 2.814, P = 0.006); In the 1-week group 16 patients were cured, 16 patients were markedly effective, 7 patients were effective and 13 patients were ineffective, which showed no statistically significantdifference compared with the 2-week group (Z = - 0.565, P = 0.572) - 20 patients cured, 12 patients markedly effective, 9 patients effective and 11 patients ineffective. During the course of treatment, the incidence of adverse reactions of patients was 17.31% in the 1-week group, which was markedly higher than the corresponding 3.85% in the 2-week group (χ2=4.981, P = 0.026). Conclusion The clinical efficacy of pulsed dye laser in treating hypertrophic scar after burn at the1-week interval is equivalent to that at the 2-week interval, but the 2-week interval is more favorable for reducing scar tissue blood perfusion and can realize a lower incidence of adverse reactions. |
