          【摘要】 目的 探讨奥美拉唑联合纤维蛋白粘合剂在消化性溃疡出血中的应用效果。方法 选取 2020 年 5月至 2022 年 5 月信阳市第三人民医院收治的 86 例消化性溃疡出血患者作为研究对象, 按照不同治疗方法将其分为联合组 (43 例) 和单一组 (43 例), 联合组患者在对症支持治疗的基础上采用奥美拉唑联合纤维蛋白粘合剂治疗, 单一组患者在对症支持治疗的基础上单纯采用奥美拉唑治疗, 对比观察两组患者血清谷胱甘肽过氧化物酶(GSH-Px)、脂质过氧化物 (LPO) 水平, 胃组织内 H+-K+-ATP 酶活性及胃液酸碱度、凝血功能与临床疗效。结果 治疗 7 d 后, 联合组患者血清 GSH-Px 水平、胃液酸碱度均明显高于单一组 ( t = 3.315、2.238, P = 0.001?0.028), 血清 LPO 水平、胃组织内 H+-K+-ATP 酶活性均明显低于单一组 (t = 3.279、2.651, P = 0.002、0.010)。治疗 2、7 d 后, 联合组患者凝血酶原时间、凝血酶时间、活化部分凝血活酶时间均明显短于单一组 (治疗 2 d后: t = 2.186、2.250、2.061, P = 0.032、0.027、0.042; 治疗 7 d 后: t = 2.090、2.003、2.558, P = 0.040?0.048、0.012), 纤维蛋白原水平均明显高于单一组 ( t = 2.199、2.352, P = 0.031、0.021)。联合组患者总有效率为 95.35%, 明显高于单一组患者的总有效率 81.40% (χ2 = 4.074, P= 0.044)。结论 与单纯应用奥美拉唑相比, 奥美拉唑联合纤维蛋白粘合剂更能有效调节消化性溃疡出血患者 GSH-Px、LPO 水平,改善 H+-K+-ATP 酶活性、胃液酸碱度和凝血功能, 治疗效果更好, 临床应用价值更高。

          【Abstract】 Objective To study the clinical efficacy of omeprazole combined with fibrin adhesive in the treatment of peptic ulcer bleeding. Methods 86 patients with peptic ulcer bleeding, admitted to Xinyang Third People’s Hospital from May 2020 to May 2022 were enrolled as the research subjects and divided, according to the different treatments, into the combination group ( n = 43) and the single group ( n = 43). Patients in the combination group were treated with omeprazole combined with fibrin adhesive based on symptomatic and supportive treatments, whereas patients in the single group were treated with omeprazole alone based on symptomatic and supportive treatments. The levels of serum glutathione peroxidase (GSH-Px) and lipid hydroperoxide (LPO), the activity of H+-K+-ATPase in gastric tissues, the pH value of gastric juice, coagulation function and clinical efficacy were compared between the two groups. Results After 7 days of treatment, the serum GSH-Px level and pH value of gastric juice in the combination group were significantly higher than those in the single group ( t = 3.315 and 2.238, P = 0.001 and 0.028), and the serum LPO level and H+-K+-ATPase activity in gastric tissues were significantly lower in the combination group ( t = 3.279 and 2.651, P = 0.002 and 0.010). After 2 and 7 days of treatment, the prothrombin time, thrombin time, and activated partial thromboplastin time of patients in the combination group were all significantly shorter than those in the single group ( after 2 days: t = 2.186, 2.250 and 2.061, P= 0.032, 0.027 and 0.042; after 7 days: t = 2.090, 2.003 and 2.558, P= 0.040, 0.048 and 0.012), and the fibrinogen level was significantly higher in the combination group (t = 2.199 and 2.352, P = 0.031 and 0.021). The total effective rate was 95.35% in the combination group, being remarkably higher than 81.40% in the single group (χ2 = 4.074, P = 0.044). Conclusion Compared with the simple application of omeprazole, the combined application of omeprazole and fibrin adhesive is more effective in regulating GSH?Px and LPO levels, improving H+-K+ -ATPase activity, pH value of gastric juice and coagulation function of patients with peptic ulcer bleeding, and realize better clinical efficacy.
