【摘要】 目的 分析探讨镍钛记忆合金环抱器内固定在多发性肋骨骨折中的应用效果。方法 选取 2019 年1 月至 2021 年 12 月洛阳市偃师区人民医院收治的 60 例多发性肋骨骨折患者作为研究对象, 根据不同手术固定方式将其分为内固定组 (32 例) 和外固定组 (28 例), 内固定组患者采用镍钛记忆合金环抱器内固定治疗, 外固定组患者采用胸部护板外固定治疗, 对比观察两组患者疼痛程度、呼吸功能、临床疗效以及并发症发生情况。结果 术后第 3、7 天, 内固定组患者视觉模拟评分法 (VAS) 评分均明显低于外固定组 ( t = 2.176、6.377, P =0.034、P<0.001); 术后1个月, 内固定组患者第1秒用力呼气容积 (FEV1)、用力肺活量 (FVC) 及呼气流量峰值 (PEF) 均明显高于外固定组 (t = 3.067、2.516、3.108, P = 0.003、0.015、0.003); 术后 3 个月, 内固定组患者治疗总有效率为 93.75%, 明显高于外固定组患者的治疗总有效率 71.43% ( χ2 = 5.375, P = 0.021); 术后6 个月, 内固定组患者并发症发生率为 9.38%, 明显低于外固定组患者的并发症发生率 32.14% ( χ2 = 4.838, P =0.028)。结论 与胸部护板外固定相比, 镍钛记忆合金环抱器内固定更能有效缓解多发性肋骨骨折患者疼痛程度, 促进肺功能恢复, 减少术后并发症的发生, 临床疗效确切。 |

【Abstract】 Objective To analyze the clinical efficacy of nickel?titanium memory alloy embracing internal fixator in the treatment of multiple rib fractures. Methods 60 patients with multiple rib fractures, admitted to Luoyang Yanshi District People’s Hospital from January 2019 to December 2021, were enrolled as research subjects and were divided into the internal fixation group ( n = 32) and the external fixation group ( n = 28) according to the different surgical fixation methods they received. Patients in the internal fixation group were treated with nickel?titanium memory alloy embracing internal fixator, whereas patients in the external fixation group were treated with chest guard plate external fixation. The degree of pain, respiratory function, clinical efficacy and the occurrence of complications were compared between the two groups. Results Respectively on day 3 and 7 after surgery, the visual analogue scale (VAS) scores of patients in the
internal fixation group were significantly lower than those in the external fixation group (t = 2.176 and 6.377, P = 0.034,P<0.001). One month after surgery, the forced expiratory volume in one second (FEV1), forced vital capacity (FVC),and peak expiratory flow (PEF) of patients were significantly higher in the internal fixation group compared with the external fixation group (t = 3.067, 2.516 and 3.108, P= 0.003, 0.015 and 0.003). Three months after surgery, the total effective rate of patients was 93.75% in the internal fixation group, which was significantly higher than the corresponding 71.43% in the external fixation group ( χ2 = 5.375, P = 0.021). Six months after surgery, the occurrence of complications in the internal fixation group was 9.38%, significantly lower than 32.14% in the external fixation group (χ2 = 4.838, P= 0.028).Conclusion Compared with the external fixation with the chest guard plate, the internal fixation with nickel-titanium memory alloy embracing fixator can more effectively alleviate pain in patients with multiple rib fractures, promote lung function recovery, reduce the occurrence of postoperative complications, and achieve significant clinical efficacy. |
