          【摘要】 目的 分析湿润烧伤膏联合康复新液在低位肛周脓肿根治术后创面中的应用效果。 方法 选取2022 年 4 月至 2023 年 10 月耒阳市中医医院收治的 74 例拟行低位肛周脓肿根治术治疗的患者作为研究对象, 按照术后创面不同换药方法将其分为研究组 (38 例) 与对照组 (36 例), 研究组患者低位肛周脓肿根治术后创面采用湿润烧伤膏联合康复新液治疗, 对照组患者低位肛周脓肿根治术后创面采用凡士林联合康复新液治疗, 对比观察两组患者术后创面水肿、分泌物、肉芽组织生长与疼痛情况以及血清炎症因子水平、创面缩小率与创面愈合时间。 结果 术后 7、14 d, 研究组患者创面水肿及分泌物评分均明显低于对照组 (术后 7 d: Z = - 3.459?-3.252, P= 0.001、0.001; 术后 14 d: Z= -3.239、-3.514, P = 0.001、P<0.001); 术后 7、14、21 d, 研究组患者创面肉芽组织生长及疼痛评分均明显低于对照组 (术后 7 d: Z = -2.944、-3.022, P = 0.003、0.003; 术后14 d: Z = -2.597、-2.705, P = 0.009、0.007; 术后 21 d: Z = -3.032、-2.106, P = 0.002、0.035); 术后 5 d,研究组患者血清 C 反应蛋白 ( CRP)、降钙素原 ( PCT) 水平均明显低于对照组 ( Z = - 2.480、- 2.120, P =0.013、0.034); 术后第 21 天, 研究组患者创面缩小率明显大于对照组 (Z= -2.689, P= 0.007); 研究组患者创面愈合时间明显短于对照组 (Z= -2.414, P= 0.016)。 结论 与凡士林联合康复新液换药治疗相比, 湿润烧伤膏联合康复新液更能有效促进低位肛周脓肿根治术后创面水肿吸收, 减少创面渗出, 减轻创面疼痛及炎症反应程度, 加快肉芽组织生长及创面愈合, 临床应用价值更高。

          【Abstract】 Objective To analyze the clinical efficacy of MEBO combined with Kangfuxin Solution in treating postoperative wounds of radical resection of low perianal abscess. Methods 74 patients admitted to Leiyang Traditional Chinese Medicine Hospital from April 2022 to October 2023 for radical resection of low perianal abscess were enrolled as research subjects and divided into the study group (n = 38) and the control group ( n = 36) according to different dressingchange methods of postoperative wound. After the radical resection of low perianal abscess, the wounds of patients in the study group were treated with MEBO in combination with Kangfuxin Solution, while the wounds of patients in the control group were treated with Vaseline in combination with Kangfuxin Solution. Wound edema, secretion, granulation tissue growth, pain degree, levels of serum inflammatory factors, wound reduction rate, and wound healing time were compared between the two groups. Results Respectively on day 7 and 14 after operation, the scores of wound edema and secretion in the study group were significantly lower than those in the control group (day 7 after operation: Z= -3.459 and -3.252, P=0.001 and 0.001; day 14 after operation: Z= -3.239 and -3.514, P= 0.001, P<0.001). Respectively on day 7, 14 and 21 after operation, the scores of wound granulation tissue growth and pain in the study group were significantly lower than those in the control group (day 7 after operation: Z= -2.944 and -3.022, P= 0.003 and 0.003; day 14 after operation: Z= -2.597 and -2.705, P= 0.009 and 0.007; day 21 after operation: Z= -3.032 and -2.106, P = 0.002 and 0.035).On day 5 after operation, the levels of serum C?reactive protein (CRP) and procalcitonin (PCT) were significantly lower in the study group (Z= -2.480 and -2.120, P= 0.013 and 0.034). On day 21 after operation, the wound reduction rate in the study group was significantly higher than that in the control group (Z= -2.689, P= 0.007), and the wound healing time in the study group was significantly shorter than that in the control group (Z=-2.414, P=0.016). Conclusion Compared with Vaseline combined with Kangfuxin Solution, MEBO combined with Kangfuxin Solution in the treatment of postoperative wounds of radical resection of low perianal abscess can effectively promote wound edema absorption, reduce wound secretion, alleviate wound pain and inflammatory response, and accelerate granulation tissue growth and wound healing.
