          【摘要】 目的 分析探讨核心肌群康复训练在老年股骨颈骨折人工关节置换术后康复中的应用效果。方法 选取 2020 年 1 月至 2022 年 6 月河南省第二人民医院收治的 60 例老年股骨颈骨折患者作为研究对象, 按照术后不同康复方法将其分为试验组 (30 例) 和对照组 (30 例), 试验组患者人工关节置换术后在常规康复训练基础上行核心肌群康复训练, 对照组患者人工关节置换术后单纯行常规康复训练, 对比观察两组患者髋关节功能、疼痛程度及不良事件发生情况。结果 术后 2、3 个月, 试验组患者 Harris 评分均明显高于对照组 ( t = 6,009、5,644,P 均<0,001); 术后 1、2、3 个月, 试验组患者视觉模拟评分法评分均明显低于对照组 ( t = 2,454、3,587?3,473, P= 0,017、0,001、0,001); 试验组患者术后不良事件发生率为 3,33%, 明显低于对照组患者的术后不良事件发生率 20,00% (χ2 = 4,043, P= 0,044)。 结论 与单纯行常规康复训练相比, 在其基础上行核心肌群康复训练更能有效提高老年股骨颈骨折患者人工关节置换术后髋关节功能, 降低患者疼痛程度与不良事件发生风险。

          【Abstract】 Objective To study the clinical efficacy of core muscle rehabilitation training in the rehabilitation of elderly patients after the artificial joint replacement for femoral neck fracture. Methods 60 old patients with femoral neck fracture admitted into The Second People’s Hospital of Henan Province from January 2020 to June 2022 were enrolled as research subjects and divided, according to the different postoperative rehabilitation methods they received, into the experiment group (n = 30) and the control group (n = 30). After the artificial joint replacement, patients in the experiment group received the core muscle rehabilitation training based on the conventional rehabilitation training, whereas patients in the control group received the conventional rehabilitation training alone. The hip joint function, pain degree, and occurrence of adverse events were compared between the two groups. Results Respectively on month 2 and 3 after surgery, the Harris scores of patients in the experiment group were significantly higher than those in the control group ( t = 6.009 and 5.644, both P < 0.001); respectively on month 1, 2 and 3 after surgery, the visual analogue scale scores of patients in the experiment group were significantly lower than those in the control group (t = 2.454, 3.587 and 3.473, P= 0.017, 0.001 and 0.001); the incidence of postoperative adverse reactions of patients in the experiment group was 3.33%, being much lower than 20.00% in the control group ( χ2 = 4.043, P = 0.044). Conclusion Compared with the simple conventional rehabilitation training, the core muscle rehabilitation training in combination with the conventional rehabilitation training is more effective in improving hip joint function recovery of elderly patients after artificial joint replacement for femoral neck fracture, and reducing their pain degree and occurrence risk of adverse reactions.
