【摘要】 目的 分析下肢静脉曲张性溃疡应用超声引导下射频消融联合湿润烧伤膏治疗的效果。方法 选取2021年4月至2023年4月华中科技大学同济医学院附属协和医院收治的 100 例下肢静脉曲张性溃疡患者作为研究对象,按照不同换药方法将其分为研究组(50例) 与对照组(50例)。研究组患者采用超声引导下射频消融联合湿润烧伤膏治疗,对照组患者采用超声引导下射频消融联合碘伏治疗,对比观察两组患者疼痛程度、炎症因子水平、临床疗效及创面愈合时间? 结果 治疗第5、10 天,研究组患者创面疼痛程度及血清肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6 (IL-6)水平均明显低于对照组(治疗第5天: t = 5.300、3.468、4.891、3.182,P<0.001、P<0.001、P< 0.001、P = 0.002; 治疗第10天: t = 6.761、5.711、11.420、3.900,P均<0.001); 治疗第14天时,研究组患者临床疗效明显优于对照组 (Z= -2.865,P= 0.004); 研究组患者创面愈合时间明显短于对照组(t = 5.971,P<0.001)。结论 超声引导下射频消融联合湿润烧伤膏治疗下肢静脉曲张性溃疡,可明显降低创面疼痛及炎症反应程度,加快创面愈合,疗效显著。 |

【Abstract】 Objective To analyze the clinical efficacy of ultrasound?guided radiofrequency ablation combined with moist exposed burn ointment in the treatment of varicose vein ulcers of the lower extremities. Methods 100 patients with varicose vein ulcers of the lower extremities admitted to Union Hospital of Tongji Medical College of Huazhong University of Science and Technology from April 2021 to April 2023 were enrolled as research subjects and divided into study group (n =50) and control group (n = 50) according to different dressing change methods. Patients in the study group were treated with ultrasound-guided radiofrequency ablation combined with moist exposed burn ointment, while patients in the control group with ultrasound-guided radiofrequency ablation combined with povidone iodine. Pain degree, inflammatory factor levels, clinical efficacy, and wound healing time were compared between the two groups. Results On days 5 and 10 of treatment,the wound pain degree and levels of serum tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), and interleukin-6( IL-6) of patients in the study group were significantly lower than those of the control group (day 5 of treatment: t = 5.300, 3.468, 4.891 and 3.182, P<0.001, P<0.001, P<0.001, P = 0.002; day 10 of treatment: t = 6.761, 5.711, 11.420 and 3.900, all P< 0.001). On day 14 of treatment, the clinical efficacy of patients in the study group was significantly better than that of the control group (Z= -2.865, P = 0.004); and the wound healing time of patients in the study group was significantly shorter than that of the control group ( t = 5.971, P< 0.001). Conclusion Ultrasound-guided radiofre-quency ablation combined with moist exposed burn ointment can significantly reduce degree of pain and inflammatory responses in treating varicose vein ulcers of lower extremities, accelerate wound healing, and the clinical efficacy is definite. |
