          【摘要】 目的 分析湿润烧伤膏联合远红外线在 Wagner 2 ~ 3 级糖尿病足中的应用效果。 方法 选取 2021年 10 月至 2023 年 10 月中国人民解放军联勤保障部队第九八八医院收治的 120 例 Wagner 2~ 3级糖尿病足患者作为研究对象, 按照不同治疗方法将其分为观察组(60例)和对照组(60例), 观察组采用湿润烧伤膏联合远红外线治疗患者局部创面, 对照组单纯采用湿润烧伤膏治疗患者局部创面, 对比观察两组患者患侧神经传导速度、足背动脉血流动力学指标及创面愈合情况。结果 治疗 28 d后, 观察组患者患侧腓肠神经与腓浅神经感觉传导速度、腓总神经与胫神经运动传导速度以及足背动脉峰值流速均快于对照组( t = 5.240、5.317、2.541、4.728、5.647, P<0.001、P<0.001、P= 0.012、P<0.001、P<0.001),足背动脉血流量多于对照组、血管内径大于对照组 (t = 9.446、5.778, P 均<0.001), 创面愈合效果优于对照组 (Z = -2.266, P = 0.023)。结论 湿润烧伤膏联合远红外线可明显改善Wagner 2~ 3级糖尿病足患者神经功能及足背动脉血流动力学状态, 提高创面愈合速度。

          【Abstract】 Objective To analyze the clinical efficacy of MEBO ointment combined with far infrared ray in treating Wagner grade 2-3 diabetic foot. Methods 120 patients with Wagner grade 2-3 diabetic foot admitted to 988th Hospital of PLA Joint Logistics Support Force from October 2021 to October 2023 were enrolled as research subjects and divided into study group (n = 60) and control group (n = 60) according to different treatment methods. The local wounds of patients in the study group were treated with MEBO combined with far infrared ray, while the local wounds of patients in the control group were treated with MEBO alone. The nerve conduction velocity, hemodynamics index of dorsal pedis artery, and wound healing condition in the affected side of the patients were compared between the two groups. Results After 28 days of treatment, the sensory conduction velocities of sural nerve and superficial peroneal nerve, motor conduction velocities of the common peroneal nerve and tibial nerve, and peak velocities of dorsal pedis artery of the affected side of patients in the study group were faster than those of the control group (t = 5.240, 5.317, 2.541, 4.728 and 5.647, P<0.001, P<0.001,P = 0.012, P<0.001, P<0.001). The blood flow of dorsal pedis artery of patients in the study group was more than those of the control group, and the vascular diameter was larger than that of the control group ( t = 9.446 and 5.778, both P< 0.001). The wound healing effect in the study group was obviously better than that in the control group (Z= -2.266, P =0.023) Conclusion MEBO combined with far infrared ray can significantly improve the nerve function and hemodynamics status of dorsal pedis artery in patients with Wagner grade 2-3 diabetic foot and accelerate wound healing.
